Part Time Social Worker or Social Service Worker
Grove Park Home is a not for profit and charitable organization that is comprised of 143 Long Term Care Beds, a boutique retirement property, and an Adult Day Program.

We are currently hiring a part time Social Worker or Social Service Worker to support and foster a holistic and resident-centered approach to care in our long term care home.
The Social Worker/Social Service Worker provides advocacy, education and psychosocial support to residents and their families, and works collaboratively with all members of the interdisciplinary team.
This position will also include playing a supportive role for staff as it relates to mental health and well-being.QUALIFICATIONS & EDUCATION:
- Postsecondary education in Social Work or Social Services Worker Program
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers
- Knowledge of relevant policies, procedures, legislation and regulations applicable to the longterm care sector, including the Fixing Long-Term Care Act, 2001 and the Health Care Consent Act, 199
- Specialized knowledge in gerontology, dementia care, palliative care or geriatric mental health
- Two (2) years of relevant social work experience, preferably in a long term care setting
- Proficiency in a second language is an asset
RESPONSIBILITES INCLUDE:
- Conduct screening and assessments of residents' psychosocial functioning using a resident centred approach
- Develop and implement intervention plans aimed at addressing psychosocial concerns
- Autonomously manage the Social Work or Social Services referrals and caseload
- Provide ongoing assessment of needs, mediation and advocacy for resident wellbeing in collaboration with Grove Park Home's interdisciplinary team
- Offer support to residents and families in significant areas such as loss, adjustment, disease understanding, coping and system navigation
- Engage outside agencies and community groups as resources to offer support to residents/families and/or staff
- Document professionally and appropriately in the resident medical record, and collaboratively participate in the development of comprehensive care plans
- Participate in care conferences for residents on the social work caseload
- Clarify resident representatives based on Powers of Attorney or the SDM Hierarchy as needed
- Provide education, counselling and referrals for residents, families and staff regarding mental health, addictions and wellbeing
- Assist in the development of effective communication between residents, families and Grove Park Home
- Participating in Continuous Quality Improvement (CQI) activities
- Co-Chair the Grove Park Home Diversity Committee
Hours in this position are flexible working 20-30 hours per week.
