    The Case Manager I will be responsible for a broad range of tasks and responsibilities, which fall under the role.

    This individual will assume a lead patient advocate role and will coordinate functions to ensure patients actively involve themselves in those activities which will promote acquisition of skills to enhance their ability to function more independently and successfully in the community upon resolution of legal conflict.

    Specific case management tasks are assigned relative to the program service delivery and each case manager will possess efficient knowledge and skills to provide adequate services related to the assessment and treatment of forensic mental health patients and to assist and consult with staff representing other disciplines in preparation of appropriate treatment programs for each patient.

    This individual will be under the clinical supervision of the Chief Social Worker on the program.

    Essential Job Functions:

    Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.


    1.

    Coordinates patient advocate functions to include, but not limited to MHA visits, visitation lists acquisition of social security cards, eyeglasses, etc.

    Actively participates as a member of the Interdisciplinary Treatment Team to promote a unified, effective team approach using the expertise of all team members in order to assure high quality patient care.

    Actively participates in psychiatric rehabilitation activities, through the Social Learning/Diagnostic System, to create a positive social learning environment for patients served.

    4. Conducts Didactic classes with assigned patients according to approved level of clinical expertise and the needs of the program. 10%

    Actively participates and/or serves in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.

    6. Provides staff coverage when required. 5%

    Knowledge Skills Abilities:

    1.Assume the responsibility to act professionally on his/her judgment and initiates requests for professional support on a consultative basis as needed.


    2.Possess an ability to work cooperatively and effectively with the other program staff.

    3.Possess an ability to communicate effectively using both oral and written methods of communication

    4.Possess a general knowledge of available community resources and the means to access them effectively.

    5.Demonstrate an ability to read and comprehend clinical assessments, identify pertinent information, and be able to integrate these reports and information into treatment plans.
