Reporting to the Patient Flow Manager, the Patient Flow Coordinator plays a key role between Providence Care's staff, physicians and external partners in ensuring seamless, quality and efficient patient flow activities.

The Coordinator responds to inquiries from key stakeholders directly and/or indirectly and assists the Manager by researching and providing requested data or background information, as requested.

The Coordinator problem solves barriers to transitions in care by working closely with the Access to Care and Transition and the clinical program teams to identify solutions.

The Coordinator completes daily, weekly and monthly summaries provided to relevant 'leaders in the hospital and is fully informed of issues and challenges that require action.


Skills and Qualifications Required:


  • Baccalaureate degree in Nursing, preferred or an equivalent combination of education and experience
  • Master's degree in health studies from a recognized educational institution, preferred
  • Current, unrestricted registration with the College of Nurses of Ontario
  • Two years of experience in providing clinical practice in a hospital setting with the relevant population. Intake and discharge planning functions preferred
  • Experience with electronic patient record systems

Position Responsibilities:


  • Leads Bed Utilization Meetings and other key meetings, as assigned, to facilitate communication and coordination related to patient flow
  • Reviews Daily Dashboard and clarifies data quality issues
  • Triages inpatient referrals for appropriate and timely review by respective inpatient Access to Care and Transition specialist
  • Problem solves barriers to transitions in care at admission, transfer and discharge
  • Provides daily and weekly summaries to Providence Care leadership
  • Review trends, variances and reasons for variances for admissions, transfers and discharges
  • Reviews trends and reasons for conversion to Alternative Level of Care (ALC) for active patients
  • Participates in joint PC/SELHIN monthly ALC review meetings and provides support to teams, as required
  • Supports the implementation of ALC mitigation strategies, as assigned by Manager
  • Ensures the ALC signoff protocol is followed and approves ALC designation; as assigned by Manager
