West Park Healthcare Centre helps patients get their lives back by providing specialized rehabilitative and complex care after a life‐altering illness or injury such as lung disease, amputation, stroke, and traumatic musculoskeletal injuries.


West Park is building a new hospital and transforming its 27-acre site into an integrated campus of care, enabling the hospital to evolve its rehabilitative programs to meet Ontario's future healthcare needs.


Our current CAVC unit is expanding and we are adding 23 beds to our existing 36 bed unit plus an additional 4 beds dedicated to Transitional Home Ventilation (THV) program as well as a 6 bed Enhanced Living Service unit.

Patients are ventilator dependent requiring total patient care. Patients are cared for by an inter-professional team, of which Respiratory Therapists are key. Please join us on this journey forward as West Park continues to expand.


Qualifications:


  • Bachelor's degree in a clinical discipline
  • Registration with appropriate regulatory college or eligibility for same
  • Member of relevant professional association/network preferred
  • Minimum of 4 years clinical professional background
  • Demonstrated clinical assessment skills including the ability to assess clients from preadmission to post discharge
  • Experience with patients on mechanical ventilation is an asset
  • Knowledge of external agencies and resources
  • Demonstrated ability to partner and work effectively with all levels of staff, the interprofessional team, physicians, patients, families and external agencies
  • Demonstrated ability to deal with complex mental health and behavioral issues
  • Ability to maintain effectiveness in varying environments and with different tasks, responsibilities and people
  • Demonstrated knowledge of and ability to provide patient centred care. Demonstrated organizational, problem solving, interpersonal, coaching, negotiation and facilitation skills
  • Strong organizational skills to work independently in an unstructured, demanding environment
  • Demonstrated reliable work habits
  • Ability to work a flexible schedule
  • Strong Windows based computer skills e.g. Microsoft Office Suite
  • Fluency in a language other than English an asset preferred
  • Experience in the clinical setting for which the individual has been hired preferred
  • Evidence of satisfactory attendance record
  • An employee's disciplinary record will be reviewed as part of the assessment of qualifications

  • Should be aware of conditions and measures that may affect the safety of patients, residents, students, employees, volunteers, physicians and visitors

Accountabilities:


  • Working in collaboration with clinical coordinator Long Term Vent Strategy (LTVS) assess patients for appropriate admission to service and liaise with physicians and team.
  • Manage waitlist and occupancy rates for the service in collaboration with clinical coordinator LTVS
  • Liaise with referring agencies and provide ongoing support/education during the preadmission phase.
  • Coordinate the care of clients from preadmission to postdischarge while meeting length of stay and occupancy rate targets.
  • Serve as the coordinating contact between patients, family, team members and referring agencies.
  • Refer and coordinate consultant / discipline involvement as needed.
  • Support collection of clinical utilization data in collaboration with Service Manager.
  • Complete clinical workload
  • Provide resource clinical leadership for the interprofessional team.
  • Provide leadership in program evaluation, program development and research as appropriate.
  • Participate in internal and external committees.
  • Partner with Service Manager to ensure clinical service goals are addressed.
  • Ensure that clinical care issues / concerns are addressed in a timely and effective manner.
  • Ensure that care is patientfocused with an emphasis on culturally appropriate care.
  • Perform other duties as assigned.
