Home and Community Care Support Services Champlain is one of 14 Home and Community Care Support Services organizations in Ontario.

We are dedicated to ensuring our more than 64,000 patients annually receive the highest-quality health care and services at home.

Our patient-care coordination teams help develop care plans for people of most ages, focused on maintaining each patient's independence and dignity.

We also provide long-term care home placement services, and facilitate our patients' access to the community-based services they need.


Position Summary:


Under the general direction of the Manager, Home and Community Care and in accordance with HCCSS Champlain standards, legislation, policies and guidelines, the Rehabilitation Assistant is responsible for providing therapy assistance and administrative support to Physiotherapy and/or Occupational Therapy staff.


Primary Responsibilities:

Direct Client Intervention

  • Implements treatment plan with patient under the direction of a registered therapist
  • Assists registered therapists with patient assessments and treatment programs, in accordance with the College of Physiotherapists of Ontario (CPO) and the College of Occupational Therapists of Ontario (COTO) guidelines
  • Completes and maintains documentation regarding patient intervention in accordance with HCCSS Champlain policies and procedures and CPO/COTO guidelines
  • Meets with the registered therapist regularly to discuss patient treatment plan and progress
  • Researches assessment, diagnosis, and intervention tools/aids/materials to assist the registered therapist in delivering patient services
  • Carries out all duties in accordance with HCCSS Champlain policies and procedures and Guidelines for the Use of Supportive Personnel developed by CPO and COTO
Equipment and Supply Management

  • Maintains equipment and supplies on a regular basis by inspecting, cleaning, and sanitizing equipment used by clients and by scheduling equipment calibration/repairs with vendors
  • Installs assistive equipment, and constructs therapeutic tools/devices for intervention, as required by the therapist
  • Maintains organization of storage areas for therapy equipment and supplies to ensure that items are easily accessible
  • Monitors inventory levels, and orders equipment and supplies as necessary, to ensure a sufficient level of inventory is available
Administrative Support

  • Prepares and organizes materials, documents, and activities for patient intervention as requested by the therapist
  • Assists therapist with documentation as required
  • Creates and maintains patient files in accordance with HCCSS Champlain policies and procedures
  • Organizes, files, and maintains equipment catalogues and educational resources on a regular basis

Education and Experience:


  • Graduation from an accredited Physiotherapist and/or Occupational Therapist Assistant program
  • Related rehabilitation and/or community care experience normally attained over a period of two years
  • Experience in Stroke and/or Neuro rehabilitation will be considered an asset

Knowledge:

Subject Matter Knowledge

  • Knowledge of equipment, modalities, materials, and intervention processes used in the provision of occupational therapy and physiotherapy services
  • Familiar with rehabilitation principles and clinical terminology used in interventions
  • Knowledge of applicable COTO and/or CPO professional guidelines
LHIN Knowledge

  • Knowledge of health care system and the role of the LHIN within the system
  • Knowledge of LHIN operations, quality, risk, and performance management principles
  • Knowledge of relevant health care legislation
  • Knowledge of LHIN policies, procedures, strategic direction and goals
  • Understanding of LHIN partners and contracted service providers in the community
General

  • Ability to accurately complete required documentation, reports, and forms
  • A valid driver's license and access to a vehicle is required

Skills and Abilities:


  • Ability to work collaboratively with registered therapists and accept work direction when assisting with patient interventions
  • Strong interpersonal skills to work with diverse patient groups with varying levels of comprehension and language capability
  • Ability to deal with demanding interpersonal situations and respond with good judgment and understanding
  • Ability to handle sensitive and confidential information in a discreet and professional manner
  • Ability to effectively exchange information with a variety of internal and external contacts
  • Ability to handle complaints from patients, their family members/caregivers, and consult with higher authority when appropriate
  • Vulnerable Sector Check completed within the last six months
  • Fluency in English and French required

Pay rate:

Wages are in accordance with the collective agreement between Home and Community Care Support Services Champlain and
OPSEU and starting salaries range from $29.999 /hr.
