CHATS is seeking a passionate, organized, and people-focused Client Care Coordinator – In-Home to support the delivery of high-quality in-home services for seniors through comprehensive assessments, documentation, coordination, and collaboration across interdisciplinary teams.
Responsibilities
- Conduct timely, comprehensive client needs and risk assessments,
- Collaborate with Client Care Supervisors to identify clients or situations requiring priority home visits,
- Provide oversight and support for priority waitlists and assessment processes.
