Description

Qualifications (Education & Experience)

  • Post secondary education in a related field is required (Ex. Counselling, Social Work, Psychology, Human Justice, Education)
  • Minimum 2 year front line youth care experience
  • Conflict Resolution, Crisis Intervention, & DPVS
  • ASIST, Mental Health First Aid, & Suicide Prevention Training
  • CPR/First-Aid (B)
  • Preference will be given to applicants with training and experience in
  • Developing, implementing, and overseeing case planning & case management
  • Individual and case advocacy
  • Preference will be given to applicants experience in working with
  • Ministry of Social Services, Child & Family Services, & Community Resources
  • Youth with exceptionalities, gang involvement, generational Trauma/PTSD/CPTSD
  • Indigenous populations, vulnerable populations
  • Individuals experiencing current/previous exploitation
  • Individuals managing substance use and/or mental health concerns

Requirements (Skills & Competencies)-

Duties & Responsibilities - Primary Liaison KPIs

Case Management Support

  • Ensure that all youth in Transitional Housing have comprehensive, holistic and up to date Case Plans and that the Case Management Plan on Apricot is completed/ kept current
  • Ensure Case Plans are being communicated to all staff working with the youth and that there is open communication back to the CPC about the youth
  • Ensure rundowns are happening between Case Plan Coordinators at the beginning and end of their shifts and with the larger team as needed
  • Review and update youth case plans and case management documents daily
  • Facilitate case planning conversations around connecting youth to appropriate resources and services in the community
  • Stay current on the community resources available to youth in the community. This may involve reaching out to community groups to make connections and learn about their services to bring back to the Case Plan Coordinator team
  • Oversee placements in Short and Long Term housing, liaising with MSS Placements Coordinator and SCP CPC around Intakes and Discharges
  • Ensure case conferences are occurring as per program manual guidelines. Provide assistance with case conferences as needed.
  • Support CPC in advocating for youth's needs as needed
  • Ensure risk assessments and routine, wellness and safety checks are being done appropriately. Advocate/liaise with MSS about getting additional 1:1 support for high risk youth if needed.
  • Ensure documentation is completed by CPC (including case notes and IDRS etc.)
  • Ensure Case Plan Coordinators or following through on meeting with MSS, CRT and other youth appointments and doing what is needed to support the youth in the best way possible
  • Provide coaching/ support in CPC dealing with challenging behaviours presented by youth

Leadership

  • Coach/ provide support to the CPC to be successful in their role and team and youth relations
  • Do weekly Check Ins with each direct report to support their professional development, sound board and provide emotional support as well
  • Complete a Professional Development Plan with each direct report and support their completion of it and growth throughout the year, documenting progress, action plans, check ins and concern.
  • Facilitate regular CPC meetings to discuss cases as support one another in best supporting the youth
  • Work with the Quality of Care Director to ensure the Case Plan Coordinators work is in alignment with quality care standards both within SCP and MSS
  • Provide direction for CPC as needed, and liaise between CPC, Senior SCP management and MSS for exceptional cases or circumstances
  • Respond to crisis situations and provide direction, support and debriefing support for CPC; arranging additional support supports as needed
