The Opportunity:

The Resident Care Coordinator is responsible to the Executive Director or the Director of Care.

They ensure and support the implementation and evaluation of the resident care services in accordance with applicable provincial standards, guidelines, and regulations to ensure that the highest standard of resident care is delivered and maintained at all times.

Key Responsibilities:


  • Employees and Partners: Support leadership to assist with ensuring and sustaining an atmosphere of safe, quality resident and family centered care
  • Supervise, direct, evaluate team members.
  • Lead the completion of accurate and timely MDS and associated assessments
  • Completes and advises in the development of Resident Care Plans.
  • Is accountable for overseeing the movein process of residents in collaboration with the Community Placement Coordinator for available beds, strategically selecting potential residents while balancing multiple variables (e.g. safety, roommate suitability, space considerations)
  • Ensures that bed vacancies are filled in a timely manner (target is 5 business days or less) to support full occupancy. Reduces delays through efficient movein processes.
  • Ensures Resident Annual Care Conferences are scheduled and attends care conferences
  • Ensures a high standard of performance and a strong resident service orientation by providing regular feedback and ensuring compliance with all policy, procedures, and standards
  • Participates in quality improvement activities to support resident care i.e. rounds/huddles
  • Keeping track of/scheduling Codes of the Month and Inspections (for PIR)

Qualifications:


  • 2+ years' experience in geriatrics, rehabilitation or long term care at a supervisory or leadership level
  • Current registration and certificate of competences with the BCCNM as LPN.
  • Experience in a multiunion environment (an asset)
  • Experience with continuous quality improvement/ risk management processes.
  • RAI/MDS practical experience
  • Priority setting/time management skills
  • Education or certification in leadership and conflict management an asset
