About the Clinic:
The Short Term Case Management and ReLinc (Case Management) Clinics at THP provide person-centred, specialized mental health care for people with persistent mental illness, in the community.
ReLinC
ReLinC Services is a community-based case management service for people aged 18 and over, with a history of serious and persistent mental illness, often a psychotic disorder.
Case Managers work in partnership with clients to develop the skills and supports they need to live successfully in the community.
Short Term Case Management
STCM provides time-limited and task-oriented case management to individuals being discharged from the Inpatient Psychiatry Unit who are diagnosed with a psychotic or mood disorder.
The intent of these referrals is to meet the short-term needs of individuals experiencing increased levels of recidivism as illustrated through their contact with the THP Emergency Department, as well as our Inpatient Programs.
The purpose of this program is to assist the client's reintegration into the community through acquisition and linkages of needed and appropriate community supports and services.
Primary Team Leader Responsibilities:
- In collaboration with the team and manager, organize and prioritize daytoday operations of the team, staffing, scheduling and resource utilization
- Round, monitor and address patient and family satisfaction with care
- Facilitate staff orientation and identify staff learning needs and appropriate resources in collaboration with program Educators & Manager
- Support the accountability of team members to the Manager with regards to patient care, professional conduct and performance
- Participate in hiring, orientation and performance evaluation of staff
- Liaise with members of interprofessional team (inpatient and outpatient and Emergency Department) to ensure excellence in patient care
- Regularly completes chart reviews and audits and followups to ensure adherence and compliance to standards of care
- Assists in the development of effective interpersonal relationships among interprofessional team through coaching, mentoring and facilitating difficult conversations
- Support patient flow, discharges and transfers of patients through a transparent, equitable and standardized process
_In addition to the above, this position will hold a reduced allocation of clinical time._
Primary Clinician Responsibilities
- Responsible and accountable for decisions and actions in the provision and management of patient case management.
- Coordinating follow up services with relevant community resources
- Provide written assessment with identifiable SMART goals.
- Facilitating group's specific to the recovery of individuals diagnosed with a psychotic disorder
- Refer and link clients and caregivers to a range of community resources, including housing, legal supports, health supports, meals, dropin centres, social recreational program, clothing exchanges, etc.;
- Collaborate with staff from the other organizations to ensure that services to all clients are timely, flexible and coordinated
- Liaise and collaborate with external services including: transitional care, acute care, emergency departments, rehabilitation centres, and other professionals, agencies and groups, to identify problems and possible solutions for service provision and improvement.
Key Qualifications
- Bachelor's Degree in a health related discipline with a minimum five years current Mental Health experience in the field of community case management, specifically supporting patients with psychotic disorders
- Minimum 3 years of experience in a formal or informal leadership position within a healthcare environment.
