Transitional Rehabilitation Housing Program 2 (TRHP 2) - Full-time Permanent**:

RESPONSIBILITIES


Reporting to the Manager of Forensic Services, the Social Worker will work with the interdisciplinary team to support their clients.

They will be required to participate in team meetings and provide regular updates to the manager. The Social Worker will also act as a liaison between related CMHA Programs and the Centre for Addiction and Mental Health (CAMH) as well as other community service providers


The aim of the Social Worker will be to assist in the development of a service care plan and linking to appropriate community services.

This will involve assessment, intake and collaborative strengths-based service care planning; identifying rehabilitation goals and risk management strategies.

Along with orientation to the program, agency and community and supporting clients to understand and adhere to the condition of their ORB dispositions.

There will be a requirement of providing on-site and community-based services and interventions, including group facilitation, one to one therapeutic support, support with ADLs, maintaining meaningful/structured day time activities, social recreational programming, psycho-education, structured relapse prevention

This role will involve Risk assessment/management, crisis prevention/intervention, monitoring, evaluation and follow-up. The Social Worker will also work to maintaining trusting, respectful and

collaborative working relationships with clients and colleagues.

There may also be some coordination of site maintenance of the White Squirrel Way site where the position will be based out of.

Salary Range:
$56,524 - $70,106
