Position Summary
Accountability
This position reports directly to one of the following:
Program Manager - CIFT, Training, Clinical Operations Manager, Manager, Aftercare
Supervises:
Social Worker Students
Corporate Responsibilities
Fosters positive interpersonal relationships with the staff, residents, and visitors, outsideagencies, government officials and related professional associations.
Maintains and adheres to organizational policies and procedures.
Ensures Legislation requirements around documentation is being followed, including Plan of Care standards
Attends work on a regular and consistent basis.
Adheres to Occupational Health and Safety Policies.
Responsibilities:
Conduct bio-psycho-social assessments, risk assessments, and provide evidenced based regular individual, family, and group therapy sessions with outpatient, day treatment and Live in Treatment clients.
Client will receive through the CIFT staff an average of 6-12 individual counseling sessions based on these modalities, and then 6-12 family counseling sessions.
CIFT will also lead evidenced based groups, including a group for caregiversAttend meetings in office, community and treatment residences including but not limited to school meetings, case conferences, treatment planning meetings, and psychology/psychiatric appointments.
Attend administrative and clinic team meetings and trainings.Will be responsible to schedule, facilitate, and lead all Plan of Care meetings, and to ensureinterdisciplinary members have completed their sections appropriately and on time.
Will beresponsible for ensuring the full Plan of Care packages are provided o the treatment team andcaregivers 3 days prior to the Plan of Care meeting, and that all signatures are obtained at theconclusion of the meeting.
Meet with Live in Treatment Supervisors and Day Treatment team regularly to coordinate each client's individual treatment plans Responsible for setting discharge dates within 3 to 6 months of admission and ensuring clients are transitioned safely back to the community within that timeframeComplete required documentation including assessments, standardized tools, session notes, treatment plans, Serious Occurrence Reports, and disclosures to CAS s in a timely manner.
Liaising with clients and their parents/guardians as well as other professionals such as school personnel, probation workers, and Children's Aid workers.
Provide intensive individual and family counselling sessions, transition/discharge support, and outreach services to ensure coordination and delivery of services to meet the needs of clients.
Attend weekly or biweekly individual supervision with Clinical Supervisor.Be available to work 2-3 evenings per week.
Other duties as assigned.
Qualifications
A Masters of Social Work (MSW) or equivalent education is required.
Minimum 2 years of relevant post qualification experience.
Membership in good standing with the Ontario College of Social Workers and Social ServiceWorkers (OCSWSSW) or the College of Registered Psychotherapist of Ontario (CRPO).
Clinical experience in providing bio-psycho-social assessments, risk assessments, and evidenced based individual and family therapy to children, adolescents and/or their families.
Clinical experience providing evidenced based group sessions to children, adolescents and/or caregivers is considered an asset.Excellent verbal and written communication skills.
Experience with using a variety of evidence-based/informed treatment approaches such as TF-CBT, DBT, EFFT, and/or McMaster Approach to Family Therapy.
Experience in working in home and/or in the client's community preferred.Evening availability required.
Valid driver's license with clean driving record is required.
