The Change Management Lead will play a critical role in ensuring the successful implementation and adoption of a large scale, multi-year program to redesign SE Health's homecare service.

This end-to-end service redesign will be guided by emerging models of care that are enabled by digital, and will be co-designed with our employees, clients, and their families.

This transformation returns the focus of homecare to client experience and health outcomes, by designing an inherently digital service that allow our 6000+ care providers to deliver holistic and integrated care more easily to our more than 1.2 million clients annually.


The Change Management Lead will be responsible for developing and executing change management and communication plans to support the implementation.

This individual will work closely with the project team, leadership, and care providers to ensure awareness and engagement throughout the project.

You will have experience leading large and complex organizations through significant change initiatives and have hands on experience in change management, communications, facilitation, and stakeholder engagement.

You will work to drive faster adoption of change, increase end user enablement and proficiency, and drive higher utilization and sustainment of changes that impact the organization.

By joining the project team early on in the project, you will have the ability to influence and guide the organization throughout the entire change lifecycle.


This role is a hybrid full-time contract role with benefits until June 30, 2025 with possibility of conversion to full-time permanent before end date.**
Responsibilities:
Activity A:
Stewarding the Change Management Strategy and Plan


  • Leverage change management and implementation science methodologies, frameworks, and tools to support the adoption of the new homecare service across the organization
  • Assess the organization's readiness for change, identify stakeholders and their roles, and develop a plan for communicating the change to relevant stakeholders
  • Identify and plan activities to enable executive and departmental leadership alignment, including clinical, technical, and business leadership, around program goals and objectives

Activity B:
Enabling and Implementing Change


  • Conduct change impact assessments to identify potential gaps, and develop and implement interventions to address them
  • Develop a deep understanding of the case for change, the urgency of change, and how this will impact each affected stakeholder group
  • Manage a change network including development of agenda, facilitation of change network meetings, and identification of key readiness activities for change champions
  • Coach directors, managers, and other key stakeholders to enable their leadership of change management activities
  • Lead design, delivery and logistics of activities related to stakeholder engagement including managing sessions such as consultations, focus groups, workshops, presentations, and meetings with affected stakeholders
  • Develop changerelated communications strategy and integrated communication plan and oversee execution, including measuring the outcomes to assess effectiveness of the strategy
  • Provide input, document requirements, and support the design and delivery of training
  • Collaborate to develop a golive plan for user support and address any issues that may arise

Activity C:
Monitoring and Evaluating Change


  • Develops metrics, monitors and measures outcomes related to rate of adoption, taking corrective action where required
  • Prepare reports and presentations as required for leadership and committees
  • Continuously scanning the environment for ideas, best practices and emerging trends linked to managing change

Qualifications:
  • Completion of a recognized Bachelor's Degree required. Master's degree in a related field (Business Administration, Healthcare Administration, HR, Psychology) preferred
  • Five (5) to eight (8) years' experience leading and delivering change management activities on complex technology projects
  • Change Management certification (PROSCI, CCMP or other change management designation) or equivalent
  • Experience supporting change management and engagement in a healthcare environment, especially projects directly impacting clinical end users, is an asset

  • Strong facilitation, coaching, and interpersonal skills, along with a customercentric mindset
  • Excellent written and oral communication skills, including experience developing written materials and presentations for an executive leadership audience
  • Capacity to synthesize complex information about business processes and systems into clear, concise impacts and benefits that can be effectively communicated through learning, presentation, and other materials
