As a
Nurse Case Manager, you will play a role in coordinating and providing care that is safe, effective and client centered.


Responsibilities:


  • Collaborate with the care team to establish and maintain a safe, therapeutic environment and routine for our clients.
  • Handling case assignments, draft care plans, review case progress and determine case closure
  • Communicating with clients, client family members and client's medical health team to create a care plan specific to each client
  • Conducting consultations for new clients and setting up care.
  • Communicating and following up with staff, ensuring staff understand care plans and are comfortable with client care
  • Conducting staff meetings, reviewing staff performance, and conducting annual reviews of staff.
  • Dropping off and ensuring clients and staff have appropriate supplies and PPE.
  • Maintaining and completing daily documentation of all client activities and events

Salary:
$29.00-$31.00 per hour

Expected hours: 37.5 per week
