Position Summary:


JD - Mental Health & Substance Use Support Worker


In accordance with established vision and values of FRAFCA and as an integral part of a multi-disciplinary
Wellness Support team that shares perspectives regarding the value of least intrusive methods and the importance of client self-sufficiency/efficacy, the
Mental Health & Substance Use (MH&SU) Support Worker provides life skills training, coaching, advocacy, information, and support services for clients experiencing mental health, behavioural and developmental problems in accordance with an established plan of care.


As a key member of the
Wellness Support and CCH team, the MH&SU Support Worker primarily focuses on specific skills training and support needs of clients such as helping individuals to acquire relapse prevention, community adjustment, and employment skills.

In line with psychosocial rehabilitation/recovery principles, the MH&SU Support Worker facilitates client empowerment and competency development within the four domains of psychosocial rehabilitation (personal life, education, leisure, and work).

All rehabilitation, recovery-oriented and harm reduction services in their healing journey will be offered in a manner which respects individual strengths, interest, capacity and choice upholding indigenous values.


The MH&SU Support Worker monitors client goal attainment and proactively assists individuals in averting potential crises, and reports client status (including any reactions to medication) and progress to the multidisciplinary team.


The MH&SU Support Worker coordinates, plans, promotes, arranges, facilitates and evaluates specialized PSR programs such as
Complex Care Housing and
Vocational Support Programs, as required.


Duties and Responsibilities of the MH&SU Support Worker:
As part of the multidisciplinary Wellness Support FRAFCA team, the MH&SU Support Worker participates in the development, maintenance, and delivery of a client-centered plan of treatment/care, by:

  • Observing, interviewing and assessing client behaviour to identify problems and needs, soliciting client feedback, inquiries, and complaints, responding to client needs as required;
  • Encouraging and promoting independence and self-sufficiency for clients and their caregivers/families; developing short-term strategies to deal with various situations;
  • Providing feedback and suggested recommendations to the treatment plans within established guidelines;
  • Documenting the care requested and progress made in the communications books; maintains client records;
  • Reporting regularly to the program coordinator / counselor on the condition, behaviour, performance, progress, and care needs of clients and their caregivers/families.
  • Provides support, coaching and behaviour modeling for the client and their caregivers/families to assist with the development of physical, social, emotion and life-skills, including all activities of daily living, interpersonal skills, yard maintenance, housekeeping, grocery shopping, meal planning and preparation, and household and budget management.
  • Guides & teaches assertiveness and self-advocacy skills to clients, and provides the necessary coaching and mentoring. Reports resolvable advocacy situations that may contain an unidentified treatment need or element of discrimination to the Coordinator.
  • Assist the therapist / counselor in observing & supporting for appropriate medication management or provides medication reminders, in accordance with the plan of treatment/care and established policies and procedures.
  • Observes and monitors clients in accordance with plan of treatment/care, assesses and responds in potential emergency or crisis situations in accordance with established procedures. Provides timely feedback and recommendations to the Case Manager regarding the management of the crisis.
