Respond to patient referrals within timelines (internal and external)Complete a comprehensive care assessment in a timely manner,
Document all required information,
Ensure Patient and Care Provider safety by accurately assessing risk using tools provided,
Reassess patients at interim visits to ensure the service plan continues to be appropriate,
- Promote service delivery excellence through effective on-boarding mentoring coaching performance managing of field staff including performance reviews
- Participate actively in meeting or exceeding utilization retention engagement expectations of field staffcare providers
- Respond to identified staff training needs with action plans to resolve gaps minimize risk ensure service delivery excellence
- Sure all internal reporting meets or exceeds all standards expectations associated Regulatory Body
,- Sure accurate timely completion of appropriate documentation including charting
- Pensure a timely comprehensive response Incident Reporting IR
