    Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families: The Lead provides expertise and leadership regarding clinical requirements for Long Term Care and/or Assisted Living to ensure clinical and quality care standards are maintained.

    In collaboration with the Manager/Director, works with service providers, Fraser Health clinical teams and community care professionals to ensure the consistent delivery of safe and appropriate quality care to clients/tenants within Fraser Health contracted, owned and operated Long Term Care sites and/or contracted Assisted Living sites.

    Leads regional internal and external meetings to engage stakeholders, identify and resolve clinical issues, and ensure consistent delivery of service.

    Responsibilities Provides expert consultative advice to clinical leaders, health care teams, and other health care practitioners in regards to the provision of care within Fraser Health contracted, owned and operated Long Term Care sites, and/or contracted Assisted Living sites, within the parameters of service contracts.

    Leads the coordination of complex admissions; engages Home Health staff, contracted service providers, and hospital staff to develop plans while ensuring eligibility requirements and quality standards for Long Term Care and/or Assisted Living are met and maintained.

    Acts as a key resource to clinical teams and relevant Long Term Care and/or Assisted Living clinical operations. Ensures quality clinical standards are communicated and reviewed. Participates in regular site visits to review clinical indicators with Fraser Health operations and service providers. Evaluates, documents, and reports findings.

    Identifies risks and develops actions aimed at helping teams meet Integrated Long Term Care and/or Assisted Living service standards and quality indicators.

    Represents Fraser Health during clinical audits that assess the delivery of clinical services within owned and operated and contracted sites and collaborates with auditors and relevant stakeholders through the auditing process.

    Collaborates with clinical leaders, health care staff and service providers to foster an environment that is focused on quality client care and aligned with Long Term Care and/or Assisted Living regulations.

    Leads and chairs regional meetings for Fraser Health Integrated Long Term Care and Assisted Living with community care teams and service providers aimed to discuss and resolve issues, within the terms of service contracts, and ensuring consistent delivery of service.

    Acts as an advocate for patient/residents/clients and families; responds to issues regarding clinical matters as required and supports compliance of the complaint review process.

    Participates in strategic planning, clinical services development, as well as the development of clinical and operational service plans; helps lead various projects related to Integrated Long Term Care and/or Assisted Living Services.

    Coordinates, participates or facilitates research and other initiatives taking an interdisciplinary and consultative approach.

    Qualifications Education and Experience Bachelor's degree in a health related discipline plus five to seven years recent, related clinical experience including two years in a leadership role, or an equivalent combination of education, training and experience.

    Professional/Technical Capabilities Understanding of the health care system and the operations of long term care and/or assisted living facilities. Ability to retrieve, analyze and evaluate clinical and operational data. Excellent report writing skills. Ability to identify, evaluate and implement process improvements. Project management skills. Knowledge of applicable software applications including database, spreadsheet and data analysis software.
