Reporting to the Manager of Student Rights & Responsibilities (SRRO), the Case Manager provides administrative leadership to the 'Care Team', manages cases referred to the Care team, and completes a number of data management and reporting functions.

The Care team is a multidisciplinary team (7+ members) which includes Safety & Security, Counselling, Residence, and Senior Leaders at the College.

The team uses a structured and objective process of assessing individual student needs and providing coordinated interventions to students experiencing barriers and/or demonstrating worrisome behaviour.

The Manger will provide case management and risk assessment for all aspects of Care Team cases, including at-risk behaviour, hospitalizations, prolonged absences, health and mental wellness, homelessness, poverty, abuse, and lack of basic needs.


The Case Manager is also a member of the Student Rights & Responsibilities portfolio, which includes two pre-existing components that naturally collaborate.

These components include the Care team and the Student Rights and Responsibilities team.

The incumbent will also work closely with the Sexual & Gender Based Violence Response, Support Coordinator, and other members of the Student Affairs team.


Responsibilities:


Case Management

  • Provides case management support to complex, unique, and sensitive student cases for students experiencing barriers and/or demonstrating worrisome behaviour
  • Oversees intakes, triages, and assessments, and provides coordinated care and support through consultation with internal and external partners, developing individual support plans, monitoring, and following up on progress and transition planning
  • Establishes a baseline for risk utilizing structured assessment tools and persisting frameworks to respond to signs of psychological distress, including signs of suicide risk, suicide ideation, risk of harm to others, etc.

Team Leadership

  • Provides administrative leadership to a multidisciplinary care team of 7+ members through a structured process of assessing individual student needs
  • Develops and coordinates all operational components for Care Team meetings, presents case updates (baseline of risk, intervention strategies, risk assessments, progress evaluations)
  • Coordinates all care plans and interventions

Data Management & Reporting

  • Maintains a confidential database, documentation, and intake files on the incidence and referrals to the CARE Team
  • Collects, analyzes, and identifies trends and statistics for reporting to College stakeholders and external government agencies as required
  • Prepares and edits presentations, reports, and other communications
  • Recommends, writes, and revises related procedures, practices, and processes

Campus-wide Activities & Committee

  • Participates and engages with institutional campuswide internal committees/working groups, as well as external community agencies to support students' wellbeing
  • In collaboration with campuswide stakeholders, develops awareness, education, communication strategies, and marketing/educational material
  • Delivers training and education sessions on student health, mental health, and success
  • Participates in projects and activities, including support for Student Affairs initiatives, as well as other special projects and research assigned by the Manager of Student Rights & Responsibilities

Qualifications:


  • Master's Degree in Student Affairs, Education, Social Work, or a related field required
  • Minimum three years of professional experience in a student conduct or case management role within a postsecondary education or closely related setting
  • Affiliation and certification with HECMA (Higher Education Case Managers Association), NaBITA (National Behavioral Intervention Team Association), and/or CACUSS (Canadian Association of College and University Student Services) are considered an asset
  • Knowledge of current best practices, trends, and issues related to student development, students at risk, and case coordination
  • Demonstrated experience building and fostering relationships with students, colleagues, and departments
  • NaBITA Care Team Best Practices Training
  • SIVRA35 or other threat assessment tools
  • Certification in Mental Health First Aid, safeTALK, and ASIST
  • Experience working as part of a case coordination team and experience navigating complex student situations
  • Demonstrated knowledge and experi
