Five Counties Children's Centre is a not-for-profit outpatient paediatric rehabilitation facility which provide therapy and developmental services and supports for over 6000 children and youth annually with physical, communication and/or developmental issues.


We serve the communities of Peterborough, Northumberland, City of Kawartha Lakes and Haliburton counties with offices in Peterborough, Lindsay, Cobourg, Campbellford, Haliburton and Minden.

Across these 6 sites, Five Counties employs approximately 140 staff who provide supports to clients, their families and community partners at the Centre, schools, daycares and in the community.

In addition to work completed at identified sites, staff also work remotely from home, schools, daycares and client homes. As a Health Information Custodian employing regulated health professionals, the Centre is legislated by the Personal Health Information Protection Act

We are seeking an experienced Project Manager to assist with the migration of our client information system.

This
full-time (35 hours/week),
contract position (six months with a possible extension of an additional six months) is located in
Peterborough, Ontario, with an expected start date of
January 30, 2023.


Position Summary:


The Project Manager will plan and implement the transition from the Centre's current electronic Client Information System to a new client information system.

The software will support families across our catchment area and staff at all sites.


Key Duties and Responsibilities:


  • Plan the implementation, including change management processes to transition systems.
  • Develop a detailed project schedule, resource plan, and status reports, managing task assignment to project resources and reporting on and managing project scope throughout the lifecycle of the project
  • Define goals and track progress towards those goals for change management, data migration tasks, implementation and training
  • Review project deliverables on a weekly basis via status for accuracy and apprise management of risks, issues and recommend mitigations
  • Interface with all areas affected by the project, including end user, vendors, families and community partners
  • Review of current processes to determine opportunities for change and improvement including: Referral to centre and referral to programs, waitlist management, capturing first assessment and first treatment, clinical documentation including outcome measures, documentation received from outside sources, use of care plans and care pathways, client history, scheduling, medical clinics, access by external partners, discharge from program and CTC and other processes as identified
  • Support the data migration process through data extraction, migration and conversion
  • Implement the new software utilizing a team of change champions determined by the organization
  • Support the creation of test scripts
  • Support the organization in creating security configurations
  • Support the organization in meeting reporting needs
  • Creation of organization specific training materials
  • Post transition follow up

Qualifications and Requirements:


  • Professional diploma/degree in computer science, data analysis, project management or information technology
  • Proven Project Management experience required

Required Work Experience_

  • Working knowledge of the design implementation and maintenance of systems hardware and systems security
  • Proven ability to deliver data migration projects
  • Proven experience for high quality and ontime project deliveries
  • Experience in business process design and/or change management
  • A minimum of 3 years related experience in data systems (database) design and structure
  • Experience in data management statistics and data integrity management
  • Indepth understanding of project management methodologies
