    Job Description:

    Job Description Attending Nurse Practitioner (NP) is a primarycare provider to the residents who works to their full scope of practice as described andoutlined by the College of Nurses of Ontario (CNO).

    The attending NP will practice bothindependently and in collaboration with other members of the health care team, residents andresident families/caregivers in the development, implementation and evaluation of resident careplans throughout various illness trajectories including:
    health promotion, injury and illnessprevention, episode and acute care, chronic disease/illness management, restorative/rehabilitativecare and palliative care.

    The Attending NP is an on-site provider, using 70% of their time toprovide direct resident care and the remaining 30% of their time for education, qualityimprovement, research, and leadership activities.


    The attending NP will contribute to:

    • Continuity of Care
    • Creation and implementation of Quality Improvement Plans
    • Research and implementation of evidence-based practices
    • Increased knowledge capacity of staff and programming to meet the needs of theresidents
    • Improved care coordination and collaboration across the care continuum
    • Internal and external committees on behalf of Wiigwas Elder and Senior CareThe Attending Nurse Practitioner (Attending NP) reports directly to the Director of ResidentCare or Administrator and is also accountable to the Medical Director for meeting the long-termcare home's (LTCH) policies, procedures, and protocols for medical services

    Qualifications:

    • Current certification with the College of Nurses of Ontario-Extended Class and a memberin good standing
    • Successful completion of Masters of Nursing Nurse Practitioner Program (PrimaryCare/Adult)
    • Current BLS certification
    • Proof of immunizations in accordance with organizational policy
    • Current criminal record check with vulnerable sector screen
    • LEAP Core Courses Completion (required)
    • Post-graduate education and experience in gerontology (recommended)
    ● Specialty certification in gerontological nursing and/or palliative care (recommended)

    Accountabilities:

    • Maintains confidentiality and acts in an ethical manner consistent with professionalexpectations and the LTC's policies and code of conduct at all times
    • Acts as a collaborative member of the primary care team, seeking appropriateconsultation/referrals to specialists and/or allied health care professionals when required
    • Engages in ongoing professional development in order to maintain clinical competenceand currency with legislation affecting their practice (i.e. CNO practice standards, FixingLTC Act)
    • Develops in-depth expertise in the care of older adults, dementia care and palliative care
    • Develops and maintains expertise in leadership
    • Holds professional libality protection
    • Maintains CPR certification for health care providers
    • Maintains registration with the College of Nurses of Ontario
    • Maintains membership with applicable professional associations (NPAO, RNAO)

    Roles and Responsibilities :

    • Perform comprehensive health assessments of LTC residents for a broad range of healthconditions and across illness trajectories
    • Ability to autonomously order and interpret diagnostic tests (including CT, MRI,Ultrasound, x-ray, laboratory investigations, etc) within the legislated NP scope, todiagnose health problems
    • Provide treatment and ongoing monitoring of acute and chronic health conditions thatinclude comprehensive pharmacologic and non-pharmacologic therapies based onresident/caregiver goals of care
    • Implement strategies to optimize resident health and quality of life in the areas of illnessand injury prevention, health promotion, health maintenance, rehabilitation andrestorative care and palliative care
    • Provide referrals to allied health care providers, specialists and specialized clinics whennecessary
    • Possess advanced knowledge, skill-set and expertise in the resident-focused care and themanagement of older adults with medical complexity, frailty and cognitive impairment
    • Initiate and participate in ongoing discussions with residents and families/caregiversregarding advanced care planning, goals of care and end-of-life care planning
    • Collaborate and maintain clear communication with all members of the resident careteam and caregivers/families
    • Document client care within the electronic medical record (point click care) within atimely manner as outlined by the College of Nurses of Ontario practice standards
    • Respond to critical laboratory results or diagnostic findings ordered and providing timelyfollow up to clients
    • Provide ongoing mentorship, knowledge-translation, and education to registered and non-registered staff in line with evidence-based guidelines to support clinical best practice
    • Consistently engage in evidence-informed practice and identify and implement research-based innovations to improve resident care
    • Regularly attend and participate in resident care conferences
    • Regularly attend and participate in professional advisory and quality committee activities(i.e. palliative committee, falls committee, skin and wound committee) as well as weeklycare rounds/huddles
    • Participate in regular development, implementation, and evaluation of facility policiesand procedures
    • Demonstrate knowledge, understanding and utilization of the Fixing Long Term Care Act,2021
