As an integral member of the Inpatient team, the Service Coordinator role is responsible for planning, organizing, directing, coordinating, and evaluating activities related to patient admission and discharge within the Program.

The primary responsibilities include but are not limited to reviewing and triaging all program referrals, including redirecting referrals to internal and external providers; conducting patient admission intakes; communicate effectively with referral sources, demonstrate flexibility in dealing with competing priorities and deadlines; performing utilization review activities to facilitate and streamline patient admission, discharge and flow; developing and monitoring quality indicators related to admission and discharge planning; implementing and coordinating a patient-centered care plan with patients, family members, significant others, and the interprofessional team; providing leadership within established community networks that serve to optimize overall health system flow, including Toronto ABI Network and GTA Rehab Network; liaising and establishing contact with members of the interprofessional team and appropriate community agencies to obtain, provide or exchange information relating to patients' psycho-social functioning and health care in the past, present and future; acting as a clinical resource for the inter-professional teams by providing information based on clinical knowledge and theory/practice related to clients with complex health care needs; establishing and maintaining a systematic review and assessment of departmental capacity; rendering services for the immediate benefit of the hospital and the community; compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients, participating in relevant research activities, program development and quality improvement projects, etc.

Qualifications:


  • Bachelor's degree with a clinical background i.e., health related field
  • Member in good standing of the respective college (as applicable)
  • Minimum of 3 years experience in a patient care environment (rehab environment preferred)
  • Experience in an inpatient environment preferred
  • Experience in the field of acquired brain injury an asset
  • Proven abilities in project planning, implementation and evaluation required
  • Excellent computer skills required (Word, MS Outlook and Excel required, Access preferred)
  • Excellent interpersonal skills
  • Knowledge of standards of practice and applicable health care legislation
  • Experience working with interprofessional teams required
  • Self directed individual with the ability to work both independently and in a group setting
  • Demonstrated leadership abilities and experience required
  • Excellent decision making skills required
  • Excellent verbal and written communication skills required
  • Excellent organizational and time management skills required
  • Successful completion of Basic Life Support (C level) Heart Saver plus (Canada)
