Description:
Responsible for strategic initiatives such as implementing Referral Outlook Scheduling system to maximize services to clients in community (timely access to care and elimination of waitlist), to implement more efficient use of clinicians as well as performance management and attendance management of frontline staff.
Instrumental in overseeing and supporting regular partnership meetings with contracted service providers and other stakeholders (acute care hospitals, primary care networks, community service agencies).
Monitors and managements performance of Direct reports as it pertains to clinical client assessments, discharge planning/facilitation, monitoring and intervention.Involved with negotiating complex client issues, Human Resources issues, addresses Patient Relations concerns and collaborates with both internal and external stakeholders.

Required Qualifications:
Bachelor's degree in a health-related discipline required, Master's level preferred. An equivalent combination of education and experience may be considered. A minimum 5 years of progressive leadership/management experience in a health-related field. Three years' experience in a community service delivery organization. Experience working with multiple organizations. Registration or certification with a professional body. Excellent interpersonal, written and verbal, and teaching skills are required.
Demonstrated knowledge and expertise in the areas of strategic planning, project management, utilization management, quality improvement, data analysis, information management, program evaluation, systems perspective and change management.
