Detailed Overview:

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Resident/Client Care Coordinator provides clinical supervision, guidance and support to nursing and nursing support staff and coordinates clinical team members within the assigned unit(s).

The Resident/Client Care Coordinator has responsibility for the day-to-day operations of the assigned area(s) and supports/participates in change management, quality improvement, risk management, policy, program, research and/or regional activities.


Responsibilities:

  • Applies established criteria and prioritizes activities related to residents/clients and ensures individual care plans are developed and implemented to meet the ongoing needs of residents/clients, in collaboration with the other clinical team members and residents'/clients' families.
  • Supervises nursing and nursing support staff, observes work and corrects where necessary, assigns and adjusts work assignments, schedules staff, monitors and recommends staff hours of work, including overtime. Coordinates the work of the clinical team overall on a daytoday basis; monitors the quality of care and the safety of residents/clients and takes corrective action as appropriate.
  • Supports quality resident/client care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out resident/client/family education regarding chronic illness, palliative care, etc; acts as a mentor and resource to staff, role modeling best clinical practices, assessing clinical skills and coaching staff and/or liaising with other health professional leaders to help them achieve established clinical performance goals.
  • Participates in or leads the investigation of complaints by residents/clients/families; follows up on complaints and implements corrective action as appropriate. Collaborates with Quality Improvement according to established protocols.
  • Initiates/leads/supports plan of care conferences and rounds for the purpose of optimal resident/client care management according to best practice; disseminates information to nursing/nursing support staff and other members of the clinical team.
  • Identifies learning requirements of staff and, in conjunction with the Manager, Educator and other members of the clinical team, provides input into the development of learning objectives and may contribute to the achievement of the learning objectives; provides clinical support and guidance and evaluates the effectiveness of learning opportunities.
