Service Coordinator (Permanent Full Time)

Job Overview
Reporting to the Program Manager, the Service Coordinator will be responsible for:

  • Daily supervision and evaluation of client programs and services, providing leadership in the efficient delivery of person-centred services to clients ensuring mission, vision and values are reflected in all aspects of service delivery
  • Notifying manager of client and staff issues or concerns that cannot be resolved immediately, or that are considered critical
  • Using relevant data and other information to develop and implement individual service plans in consultation with clients and caregivers
  • Addressing daytoday client concerns and providing support in renewal and management of client service agreements
  • Liaising with internal and external professionals and other support persons involved in client care Directly supervising front line service delivery staff by providing coaching, team building, supervision, performance reviews and performance management

Requirements:


  • Community College Diploma in Social Services, Healthcare, or other related discipline
  • A minimum of 2 years' experience in social services or healthcare with at least 1 year supervising or leading a team
  • Demonstrated experience in behaviour management programs
  • Problem solving and conflict resolution skills
  • Excellent communication skills with fellow staff and clients
  • Experience working with persons with disabilities and/or acquired brain injuries
  • General understanding of community resources and disability issues are necessary
  • Familiarity with current legislation, health and safety requirements, and Ministry of Health requirements, WHMIS and WSIB claims management
  • Comprehensive knowledge of computer hardware and software (Microsoft Suite including Microsoft 365/Teams; GoldCare experience a benefit, but not necessary)
  • Must have valid Class G drivers license and reliable vehicle. Travel will be required.
