The EMP is seeking an experienced Nurse Liaison to join our team in a critical role that requires exceptional coordination skills and a deep understanding of patient needs. As a key member of our interdisciplinary team, you will be responsible for facilitating seamless transitions between hospital, community, and home care settings.

Job Overview

We are looking for a highly skilled nurse with a minimum of three years of experience in home healthcare and acute care. Your expertise in care planning, coordination, and communication will enable you to navigate complex patient scenarios and ensure the best possible outcomes.

Responsibilities

• Coordinate patient transfers between hospitals, communities, and home care settings, ensuring timely and efficient transitions.
• Provide critical information to hospital staff regarding EMP patients, facilitating informed decision-making and effective care planning.
• Assess patient needs, identify potential barriers, and develop personalized care plans that address their unique requirements.
• Collaborate with healthcare teams, including physicians and nurse practitioners, to ensure comprehensive and coordinated care.
• Maintain accurate records and communicate effectively with patients, families, and healthcare providers to ensure continuity of care.
• Participate in quality improvement initiatives and contribute to the development of policies and procedures that enhance patient care and safety.

Requirements

• Bachelor's degree in Nursing
• Active registration with the Nurses Association of New Brunswick
• Minimum of three years of clinical nursing experience in home healthcare and acute care
• Strong understanding of the target population served by Extra-Mural Services and available community resources
• Valid New Brunswick Class 5 Driver's License and good driving record
• CPR Level C certification
• Excellent communication, interpersonal, and leadership skills
• Able to work efficiently as part of a team and demonstrate flexibility in scheduling
