Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.


With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges.

3,000 of whom are "home-bound" and frail.

We are currently seeking a
Program Manager to join our Adult Day Program.


TERM:
Full-time Permanent


LOCATION : 420 Hwy 7 E, Richmond Hill


RESPONSIBILITIES:


  • Develop annual work plan, activities and budget control; assess program performance, analyze variances and initiate program outcomes
  • Complile services & programs statistical reports for purposes of funding, evaluation and research
  • Conduct intake and assessment with service applicants within a common practice framework with standards for assessments and established protocols
  • Develop and prepare service / care plans for individual participant taken on for service
  • Initiate and implement care plan, management strategies, provide appropriate interventions and followup clients to achieve optimum social functioning and psychological wellbeing
  • Receive, review and process referrals within and beyond the agency for the program
  • Plan, implement and evaluates the health promotion and prevention programs and services for the participants
  • Assist in the development and implementation of policies, procedures and related documents in response to organization needs
  • Participate in case conference / weekly rounds to monitor and review the progress of services

QUALIFICATIONS:


  • Bachelor's degree in Nursing / Psychology / Gerontology / Social Work or equivalent
  • Additional credential is considered an asset
  • Minimum 2 years of supervisory experience in the health care sector, preferably in a community / primary health care setting working with seniors
  • Experience in developing, implementing, monitoring and evaluating senior programs and activities preferred
  • Superior time management skills, multitasking skills, and the ability to prioritize tasks with mínimal supervision
  • Strong clinical assessment skills, with knowledge of the principles and practices of home health care, community resources, and service delivery to clients
  • Excellent communication and interpersonal skills with internal team members and external contacts
  • Proficient computer skills in Microsoft Office, understanding of InterRAI Cha and AlayaCare is an asset
  • Flexible to change in work assignments and schedule (including occasional evenings and weekends)
  • Proficiency in English and a second language reflecting the community such as Cantonese or Mandarin is an asset
  • Valid police vulnerable sector check completed within one year
