POSITION TITLE:
Recreation Coordinator





REPORTS TO:
Life Enrichment and Programs Manager

***The Recreation Coordinator is responsible for implementing both small and large group programs and services pertaining to the quality of life for long-term care residents at Saint Luke's Place. The Recreation Coordinator is also responsible for one to one programs/visits for residents who choose not to participate or are unable to participate in group programs.


QUALIFICATIONS

  • Post Secondary diploma/degree specializing in Therapeutic Recreation and Leisure studies.
  • At least one year of experience in programs at a longterm care facility.
  • Computer software knowledge.
  • Good organizational and communication skills.

MAJOR DUTIES AND RESPONSIBILITIES

  • Assess therapeutic recreation and leisure needs of residents.
  • Plan, implement and evaluate activities on assigned unit.
  • Actively assist with, planning, implementing, coordinating, and evaluating special events.
  • Offer one to one visits to residents in need.
  • Welcome new residents and assist with their orientation to Saint Luke's Place.
  • Complete a new resident Recreation Admission Assessment within 21 days of admission.
  • Complete a new resident Care Plan within 14 days of admission.
  • Provide input to be brought to resident's annual Recreation Care Conferences.
  • Complete and maintain resident's RAI coding, Care Plans and Kardex on a regular basis along with quarterly and annual reviews.
  • Participate in all in-service programs and educational workshops when requested.
  • Provide regular evening and weekend programs. Provide programming on statuary holidays as per employees schedule.
  • Assist in dining room at mealtimes as needed.
  • Respects and adheres to Saint Luke's Place policies.
  • Plan, coordinate, facilitate, supervise and evaluate community outings.
  • Orientate, supervise, and provide leadership to volunteers and placement students.
  • Coordinate the décor of resident home areas to reflect seasonal, special occasions and multicultural months.

INFORMATION & COMMUNICATION MANAGEMENT

  • Effectively utilizes resident charts, Point Click Care, Activity Pro, Outlook, voice mail and external correspondence.
  • Communicate with resident families in regards to the residents social wellbeing/needs.
  • Participates as a team in the Recreation Department to assist each other with programs and problem solving. Actively brainstorm and share program ideas and solutions with teammates.
  • Participates as a team member with other departments on assigned home unit through problem solving, involvement, sharing and listening to effectively provide for resident needs.
  • Monitors and records resident participation.
  • Prepare and distribute monthly program calendars, activity booklets and event posters.
  • Observe and report any concerns to the Life Enrichment and Programs Manager.
  • Maintain accurate documentation and progress notes when necessary in Point Click Care and/or Activity Pro.
  • Requests volunteers through the Life Enrichment and Programs Manager as needed.
  • Inform Life Enrichment and Programs Manager monthly of supplies needed and food requisitions needed for programs and/or the department within a reasonable time.

  • Demonstrated ability to adapt to changing environments and resident needs, interests and behaviours.
  • Prioritizes workload based on the activities and duties of the day.
  • Keep work areas, equipment and supplies clean, safe, and tidy.

SAFETY RESPONSIBILITIES

  • Perform assigned tasks in a safe manner.
  • Promote safety among staff and residents.
  • Observe and report concerns about unsafe conditions, equipment.
  • Respond to fire emergencies in accordance with Emergency Preparedness procedures.
  • Observes and works within the guidelines of the Health & Safety Program, including proper reporting of accidents and participation in ESRTW plans (WSIA).
  • Fill out an incident report with Life Enrichment and Programs Manager if an injury occurs.
  • Reporting resident safety concerns to the home area leader/RPN as seen.

  • Recreation and Leisure Certificate (preferred)
