Description
The role of the Discharge Transition Nurse (DTN) is to contact patients discharged from the hospital with a focus on discussion of the patients' post-hospitalization plan of care. The calls do not provide medical advice. The calls may include suggestions and/or support to direct the patient to the appropriate resources or providers or to facilitate appointment scheduling. These suggestions or support are intended to assist the patient/caregiver to follow the plan of care identified in the patient's discharge summary and Patient Oriented Discharge Summary (PODS).
During these calls the discharge transition nurse will use health literacy strategies including plain language and teach-back, the information contained in the patient's PODS and a scripted dialogue to ensure that an appropriate discharge plan is in place and the patient/caregiver is able to follow this plan.
The phone calls also provide an opportunity to solicit information and collect ongoing data. Through ongoing experiential development this process may be modified over time to better meet the needs of the patient/caregiver. This position will work collaboratively with the Discharge Planner.
