The Continuing Care Centre (CCC) is committed to compassion and excellence in the provision of complex care and rehabilitation services.
Three levels of care are provided in our CCC unit that include:
- Activation / Restoration with a functional goal of progression (i.e. exercise and recreational activities) to increase and strengthen independence. The average length of stay in this program is 56 to 72 days.
- Short Term Complex Medical Management with a functional goal of stabilization and progressions (i.e. medically complex and specialized services) to help avoid further loss of function, increase activity tolerance, and progress. The average length of stay in this program is up to 90 days.
- Longterm Complex Medical Management with a functional goal of maintenance (i.e. medically complex and specialized services over an extended period of time) to maintain/slow the rate of, or avoid, further loss of function.
- Performing basic patient care functions
- Assisting patients with activities of daily living and other duties assigned.
- The PSW will be responsible for documenting on unit specific documentation forms
- Observing and reporting changes in patient conditions to appropriate team members
- Practicing infection control and safety measures in accordance with Hospital policies and procedures.
HOURS:
SKILLS & QUALIFICATIONS REQUIRED:
- Personal Support Worker Certificate from an accredited postsecondary educational institution.
- Preference will be given to those with at least one (1) year Continuing Care or Long Term Care experience within past three (3) years
- Current BCLS certification
- Current Safe Management Training Certification preferred
- Demonstrated problem solving, critical thinking, decision making, and conflict resolution skills and able to work through conflicts and differences in a respectful manner.
- Demonstrated excellence in the highest level of communication skills and customer service skills.
- Demonstrated knowledge and commitment to patient and staff safety.
- Ability to adapt and work within a constantly changing environment.
- Basic computer skills (Outlook, Cerner, etc.)
- Ability to meet the physical demands of essential duties of the position
- Demonstrated ability to attend work on a regular basis
