As a member of the Allied Health Team, the Health Guide works with vulnerable individuals who are rostered with the Guelph CHC who are facing multiple and complex barriers to accessing healthcare through system navigation and direct, one-to-one support.

This position may work at multiple sites with potential for travel to K-W and Rural Wellington.


PRINCIPAL JOB DUTIES

  • Act as a resource to clients, assisting them in navigating social service and healthcare systems.
  • Assist clients with complex needs to engage in their care plans as outlined by their care team.
  • Actively work to engage clients with their primary care providers and community support.
  • Advocate with health and social service systems/providers to ensure clients receive the care they deserve.
  • Facilitate internal and external referrals to address needs within the determinants of health to address client goals.
  • Lead case coordination between interdisciplinary team and external partners.
  • Establish and maintain knowledge of community resources and build strong relationships with external agencies and partners.
  • Maintains electronic client records according to agency and professional standards.
  • Collect and report data and participate in research upon request by Guelph CHC.
  • Assist with special projects as assigned by Guelph CHC leadership.

QUALIFICATIONS

  • University degree or college diploma in social work or other relevant discipline, or an appropriate combination of education and experience.
  • Three to five years' experience working in a communitybased organization.
  • Strong interpersonal, activelistening and advocacy skills.
  • Familiarity with local social service and health programs, services, and systems.
  • Expertise and cultural sensitivity in working with marginalized individuals including, newcomers/refugees, indigenous, LGBTQ++, streetinvolved, survivors of trauma, those living with addiction and/or mental health issues.
  • Understanding of, and alignment with, the philosophy of harm reduction.
  • Demonstrated ability to work independently and as an effective member of an interdisciplinary team.
  • Excellence in English written and oral communication. Second language is an asset.
  • Commitment to working within clientcentred, social determinants of health framework.
  • Familiarity with PHIPA and privacy law.
  • Proficiency in the use of computers and various software programs.
  • Possession of a valid Class "G" Ontario Driver's License and access to a vehicle. $2 million in liability coverage will be required for your vehicle
