    Make a difference in the lives of Albertans by playing an integral part in delivering chronic disease management programs in the Calgary Zone. The Alberta Healthy Living Program (AHLP) is a community-based chronic disease management program for adults with or at risk of developing chronic disease. AHLP aims to help clients improve their health and quality of life through patient-centred care and support. Under general supervision from the Care Manager, the Physiotherapist will work as part of multi-disciplinary teams to provide health coaching, supervised exercise programming, education and chronic disease self-management support related to increasing physical activity levels and adopting other healthy lifestyle habits. The Physiotherapy-specific roles and responsibilities will include: clinical assessment, support, and education in the areas of mobility and symptom management related to aging, disability, or chronic health conditions such as arthritis, osteoporosis, chronic pain with the aim to help restore, maintain and/or maximize movement, reduce pain, and improve quality of life; and facilitation of group education on topics such as fall prevention, balance training, pain management, safe activities and exercise modifications, and management of various chronic neuro and musculoskeletal conditions. The incumbent will work with patients through virtual formats as well as in-person at Richmond Road Diagnostic & Treatment Centre and the City of Calgary's Killarney Aquatic & Recreation Centre. Shift: Days (M,T,W).

    Description:

    As the Physiotherapist I (PT), you will provide therapeutic interventions to enable clients to achieve optimum physical function and independence in development, disease, disability, chronic conditions, illness and functional decline. You will provide intervention ( prevention, triage, screening, assessment, diagnosis, goal setting, treatment, management, education, consultation, fostering resourcefulness, case management, referral and transition support to individuals, groups, populations and communities. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. The PT collaborates with patient(s) and families in determining, implementing and evaluating appropriate rehabilitation strategies. You will participate in a variety of interprofessional teams to build awareness, enrich supports and engage in quality improvement; partnering with healthcare providers, patients, families and other stakeholders to advance services for their clients. You will contribute to quality improvement and their professional community; participates in peer mentoring; and supervises students. You may provide clinical supervision to Therapy Assistants, Therapy Aides and Provisional Physiotherapists.

    Required Qualifications:

    Bachelor's degree in Physiotherapy from an accredited university program. Current registration with the College of Physiotherapists of Alberta.

    Additional Required Qualifications:

    Minimum three years of experience, in the past five years, working in combined community and clinical settings using active therapy and exercise prescription with adults that have various chronic health conditions. Proficient in providing group facilitation and individual-focused adult education that enhance client care, health promotion, and self-management in topic areas that include fall prevention, pain and fatigue management, safe activities and exercise modifications for osteoporosis, and management of various chronic neuro and musculoskeletal conditions. Knowledge, skills, and proficiency in assessing range of motion, muscle strength testing, and functional mobility. Experience developing, planning, and implementing follow-up physiotherapy services for clients in cooperation with multi-disciplinary team members. Excellent communication skills, including verbal, non-verbal, written, and listening. Experience and demonstrated competency in working effectively and efficiently in a collaborative, consultative manner as part of a multi-disciplinary team. Demonstrated ability to establish and maintain effective and respectful working relationships with a wide variety of clients, care providers, and team members. Demonstrated ability to work independently in a rapidly changing work environment. Experience working with culturally diverse and vulnerable populations. Knowledge of information systems, databases, and computers.

    Preferred Qualifications:

    Completion of training and applied experience in chronic disease management, motivational interviewing, and health coaching. Experience providing virtual group exercise programing and one-on-one health coaching. Recent training and experience working with Connect Care.
