Trillium Gift of Life Network (TGLN) in partnership with the Ontario Renal Network (ORN), under the Clinical Institutes & Quality Programs (CIQP) portfolio at Ontario Health (OH) is leading the implementation of an integrated care value-based funding model in Ontario for the organ transplantation system.

The Transplant Funding Model Transformation Project is starting with kidney as the prototype and will be scaling costing and funding to other organs in future, adapting as needed.


  • The Manager is accountable and responsible for:
  • Developing and implementing the integrated care value-based (ICVB) funding model for transplant patients in Ontario, starting with kidney transplant patients, then expanding to all organs in future.-
  • Managing multiple workstreams to operationalize the ICVB funding model-
  • Obtaining necessary endorsement and approval through an established governance structure involving clinicians, administrators, Ministry of Health, the Ontario Hospital Association (OHA), TGLN-ORN and OH leadership-
  • Collaborating with ORN colleagues to identify intersection points in funding for kidney patients pre
- and post-transplant and aligning chronic kidney disease (CKD) and transplant funding models accordingly
  • Liaising with colleagues across OH portfolios to leverage their expertise
  • Building strong partnerships through extensive collaboration with external stakeholders including transplant hospitals, Ontario Hospital Association (OHA) and Ministry of Health
  • Leading response to the relevant recommendations from the Office of the Auditor General of Ontario's (OAGO) 2019 report on CKD Management
  • As part of this role, the Manager is accountable for the planning & administration of operations, management of resources, and engagement of key internal and/or external stakeholders to ensure operational/program activities are executed based on plans and standards, and within specified timelines and budgets.

  • Here is what you will be doing:
  • Supports the Director in developing the operational/program strategy and overall plan-
  • In coordination with the Director, determines the required frameworks, methodologies and tools to execute on the operational/program plan-
  • Oversees the day-to-day activities of multiple, large, and/or complex programs-
  • Monitors operational/program performance; provides status updates and reports on metrics-
  • Identifies improvement opportunities and escalates key issues/risks to the Director; proposes and executes solutions to address challenges or mitigate risks-
  • Monitors budget across multiple, large, and/or complex operational areas/programs, and supports the Director in managing the overall budget-
  • Manages a team of resources, including: setting performance expectations; allocating work; overseeing and ensuring the quality of outputs; measuring/reporting on performance outcomes; and performance management-
  • Manages relationships and engages a diverse set of stakeholder groups (internal and external), either to collaborate on delivery of key activities or solicit support and buy-in for operational/program activities; organizes and/or participates in committees as required-
  • Where required, deals with vendors or contractors, including negotiating contracts, managing issues and ensuring agreements are met
  • Here is what you will need to be successful:Education and Experience
  • Completion of a bachelor's or master's degree, major in business administration, health administration, health informatics, information technology, or recognized equivalent-
  • 8-10 years of overall work experience in a similar capacity is required-
  • A minimum of 4 years experience in managing a team, including setting performance objectives, and managing performance is required-
  • Experience in managing large/complex operations and overseeing day-to-day operational activities-
  • Experience in engaging and soliciting support from key stakeholders in the delivery of operational/program activities-
  • An understanding of how the overall OH strategy and direction impacts the various operational areas an asset-
  • Strong business acumen
