The Community Navigator role is to build relationships and with the assistance of volunteers to facilitate and remove barriers and through coaching encourage seniors to be self-reliant with support and accessing services.

A part of this work is the promotion of the recovery, independence, health and well-being of Creating Caring Communities (CCC) members and will work in the community in a peripatetic way.

Project Manager, Creative Caring Communities or Delegate


Responsibilities:


  • Develops Personalized Support Plans
  • Coordinates/Participates in Educational Outreach
  • Collaborates with Stakeholders
  • Leads/Collaborates on Community Engagement Activities
  • Supports/Collaborates on Advocacy Efforts and matches CCC with a volunteer.
  • Project tasks involving project planning, reporting and evaluation

Tasks:


  • Works collaboratively with others to conduct holistic client assessments that include social determinants of health, to improve overall wellbeing.
  • Works collaboratively with others to identify, organize and facilitate social activities, support groups, and educational workshops that foster wellbeing, a sense of community and help overcome social isolation among seniors.
  • Identifies unmet needs and collaborates with system partners to develop and/or deliver educational materials on health promotion, health literacy, disease prevention, and navigating systems to empower individuals with knowledge and skills.
  • Links seniors to the appropriate resource (ie: Care Coordinator, Community Relations Worker, Home Care, etc.) to facilitate referrals and access to community programs, social services, and health care options.
  • Advocates to address specific challenges and engages with Project Manager in systemlevel advocacy efforts to address issues affecting the collective wellbeing of seniors.
  • Provides project support related to data collection, reporting, planning, monitoring, outreach and evaluation as requested by the Project Manager.
  • Travel throughout the community and visiting seniors in their homes.
  • Communicates effectively with seniors and stakeholders, and maintains accurate and timely documentation of assessments, care plans, and interventions and making adjustments as needed.
  • Promotes selfcare and wellbeing, providing guidance on healthy lifestyle choices and encouraging engagement in activities that contribute to physical and mental health.
  • Completes other duties as required.


  • Minimum Grade 1

  • CCA Certification, Social Service Worker Diploma and/or other health related education
  • Comprehensive knowledge of the African Nova Scotian communities such as Lake Loon, Cherry Brook, North Preston & East Preston, their history and social structures and a member living within the black community
  • Experience with senior care, healthcare, community care
  • Project participation and/or management
  • Must have own vehicle or access to reliable transportation

Hours: 70 hours Bi-weekly (Days/Evenings)

As per the Northwood scale
