    Your Opportunity:

    Reporting to the Care Manager on the Team - Integrated Home Care, the Occupational Therapist Case Manager/Short Term works as a member of an inter-professional health care team; within the full scope of their professional practice and will be supporting clients on Seniors South team clients. The OT Interventionist /Case Manager is responsible for assessing, planning, organizing, reviewing and evaluating the care needs of clients and their caregivers/families requiring community-based services. Fosters and promotes continuity of care and co-operative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals, and other programs/organizations involved in the provision of services. Acts as an advocate for the system and the client recognizes safeguards and advances the rights and choices of the client and serves as an active liaison between the client and others internal and external to the organization. You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job. It will be an expectation for the successful candidate to complete 3 months long fulltime orientation program.

    Description:

    As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. You may also provide clinical supervision to Therapy Assistants and Therapy Aides.

    Required Qualifications:

    Completion of bachelor's degree in Occupational Therapy from an accredited university program.Registered with the Alberta College of Occupational Therapists (ACOT).

    Additional Required Qualifications:

    Please note: This position is an on-site position. Staff must report to the office daily. There is no opportunity to work from home. Completion of Master's degree in OT from an accredited university program or bachelor's degree in occupational therapy from an accredited university program and related experience. Physically able to perform all duties of the role. Excellent interpersonal and communication skills. Proficiency with Microsoft Office and other e-charting systems. BCLS-HCP required.

    Preferred Qualifications:

    Experience in geriatric clients an asset.
