We are seeking:


Intake Social Worker - Canada Summer Jobs

Full-Time Contract position - 8 Weeks (35h/week)

What will be your role?


Reporting to the Manager, Client Care and Operations, this position is responsible for perform all intake functions for the Community Support Programs including programs service coordination and support: receive, assess, and process all requests for service from potential clients and referral sources, provide information regarding the criteria for the programs and range of services offered and refer to other community services when ineligible.

Participates in VHA services and quality initiatives. All activities are conducted within the policies, procedures and standards of confidentiality of VHA.


Key tasks and responsibilities of the role:

  • Receive, assess and process service requests from potential clients and referrals
  • Conduct a thorough assessment to identify the community program best suited to the client's needs including a financial assessment to determine program eligibility
  • Develop thorough care plans from the intake level in collaboration with clients
  • Ongoing assessment and case planning through a coordinated team approach within VHA and referral sources
  • Participate and member of internal and external committee and networks.
  • Promotion, outreach and linkage to community resources
  • Conduct and present internal and external program presentations
  • Monitor, track and identify trends and service gaps
  • Administrative, planning and scheduling support
  • Participate in service, program development and quality initiatives
  • Participate in the evaluation program process

Desired Qualifications:


  • Bachelor of Social Worker or Social Services Worker diploma
  • Registered with the Ontario College of Social Workers and Social Service Workers (OCSWSS)
  • Excellent knowledge of the community health and social service networks
  • Knowledgeable in working with a range intake assessment tools, formulation and case planning skills
  • Excellent communication skills
  • Excellent problem solving and conflict resolution skills with a clientcentered approach and crisis intervention skills
  • Strong knowledge of case management theory, principles and practices
  • Knowledge of risk and family assessment framework, methods and techniques
  • Strong interpersonal and communication skills with comfort working in a high pace team
  • Strong analytical skills, combined with excellent written skills for documentation and assessment
  • Experience with project Initiatives and evaluations is an asset
  • Culturally competent and able to interact effectively with diverse stakeholders
  • Ability to handle a high volume of telephone calls and inquiries in an engaging manner
  • Strong planning, prioritizing and excellent organizational skills
  • Knowledge of the GTA area and the transportation routes
  • Satisfactory completion of technical skills testing is required
  • Working knowledge of Gold Care is a definite asset

As this role is being funded by the Canada Summer Jobs program, the following are eligibility requirements:

  • You are between 15 and 30 years of age at the start of the employment
  • You are a Canadian citizen or permanent resident or person to whom refugee protection has been conferred under the Immigration and Refugee Protection Act (foreign students are not eligible)
  • You are legally entitled to work in Canada in accordance with relevant provincial/territorial legislation and regulations
  • You have a valid Social Insurance Number at the start of employment and be legally entitled to work in Canada in accordance with relevant provin
