JOB SUMMARY:


The Coordinator - Respite and Residential works as a professional within a clinical multidisciplinary team that follows a biopsychosocial model of intervention for children, youth and adults with complex developmental, physical and emotional challenges.

This position involves directly supporting individuals as well as providing administrative supports to the program and Manager.

This includes tasks such as scheduling staff, facilitating daily operations and programming in accordance with the supported individuals, Individual Service Plan (ISP), maintaining accurate client records, completing routine reports/documentation, communicating with family/guardians, and ensuring budget compliance.

Coordinators will also be instrumental in on-boarding new employees and providing orientation and/or training to all staff.


Duties and responsibilities

Coordinator - Respite and Residential will:

  • All Coordinators will support the program through direct support to residential or respite individuals, this includes:
  • Support independence with daily living programs (toileting, feeding, grooming etc.);

Support individuals to live independently, encourage and engage individuals in meaningful activities such as being involved in leisure, recreational or work activities;:


  • Provide individuals with communication assistance, for example, typing their correspondence or obtaining information on their behalf;:

Observe, assess, and report changes in individuals' behaviours, physical abilities and activities;:

  • Perform housekeeping duties, such as cooking, cleaning, washing clothes or dishes, or running errands;
  • Provide transportation to individuals via association or public transit:
  • Schedule, coordinate and attend as necessary, medical appointments with supported individuals as required.
  • Provide personal care supports according to directions on the established care plan or assignment sheet
  • Possess and demonstrate knowledge of First Aid, CPR, CPI, Physiotherapy, WHMIS, Occupational Health and Safety Act, fire prevention and safety procedures and other areas as deemed necessary by the association.
  • Perform healthcarerelated tasks, such as administering medication or implementing physiotherapeutic activities;

  • Ensure that each person receiving support is provided with the necessary tools and guidance to cocreate an Individual Support Plan (ISP)
  • Provide people we support with the resources and opportunity to achieve the outcomes of their ISP;
  • Ensure that regular review, documentation and follow up occurs with individuals and their families in regard to meeting outcomes identified in ISPs;
  • Implementing and ensuring quality person centered programming for each individual.
  • Participate in case reviews, consult with the team caring for the supported individual to evaluate their needs and plan for continuing services.
  • Participate in case conferences with supported individual and parents/guardians as required.
  • Review records or reports pertaining to Individual Support Plans (ISP) and service delivery activities to verify details, monitor work activities and follow up with individuals in regard to meeting outcomes identified in ISPs
  • Update ISPs annually;
  • Assist in resolving employee and individual complaints, answering questions regarding policies and procedures and providing guidance in handling difficult or complex problems or in resolving escalated complaints or disputes;
  • Make recommendations to management concerning such issues as procedural or programming changes;
  • Participate in the work of subordinates to facilitate productivity or to overcome difficult aspects of work.
  • Manage change in integrated health care delivery systems, such as work restructuring, technological innovations, and shifts in the focus of care.
  • Coordinate appointments include staffing coverage as necessary;
  • Prepare activity reports to inform management of the status and implementation plans of programs, services, and quality initiatives;
  • Plan for or coordinate services, such as equipment or supply acquisition of supplies.
  • Assist in preparing schedules, deadlines, and duty assignments for staff.
  • Help develop and manage programs to empower employees to take responsibility for their jobs and goals
  • Provide guidance and orientation to new members of the team including volunteers;
  • Assist in facilitating on the job training and mentoring of staff members.
  • Provides leadership in the area of Health and Safety, to ensure homes and residents are safe and secure for supported individuals, staff and guests.
