Background


The Langley Division of Family Practice (LDFP) is a non-profit society whose members encompass family physicians, post-grad medical residents and nurse practitioners in the City of Langley and Langley Township.

The Division is governed by a Board of Directors and is managed by the Executive Director (ED).

Its members work to improve patient access to local primary care, increase local physicians' influence on healthcare delivery and policy, and provide professional support for physicians.

The Primary Care Network (PCN) is a provincial initiative that is looking to redesign and transform primary care.

A PCN is a clinical network of local primary care service providers located in a geographical area, with patient medical homes (PMHs) as the foundation.

A PCN is enabled by a partnership between the local division of family practice and health authority, along with local First Nations and Indigenous partners.

The LDFP conducts business on the unceded and traditional shared territories of the Kwantlen, Matsqui and Katzie First Nations.


Position Overview
This is a full-time permanent (1.0 FTE) in-person position with the expectation to work occasional evenings.

This position reports to the assigned Project Manager and works in collaboration with other project coordinators to support the LDFP Management Team in completing the roles and responsibilities.


Roles and Responsibilities:


1.


Project Support:


  • Support the Project Manager(s) in various tasks.
  • Schedule meetings, record decisions and outcomes, and prepare action items.
  • Gather, organize, and research resources.
  • Provide monthly and quarterly reporting and updates to the Project Manager(s).
  • Work on welcome, wellness, and recruitment strategies for Residents and FP/NP's.
  • Support the implementation of Nursing in Practice resources and clinic preparation.
  • Perform flexible job duties as needs arise.
  • Draft and format documents.
  • Ensure documents are organized, uptodate, and easy to find/access in our systems.
  • Conduct research and environmental scans as needed.

Event and Meeting Coordination:


  • Coordinate events, including booking venues and catering, and liaising with vendors.
  • Plan meetings and events, including preparing PowerPoint presentations, preevent needs, and postevent reports.
  • Capture meeting and event minutes.

Membership:


  • Support member engagement and coordinate "Ask an Expert" sessions with other project coordinators.
  • Visit and support Primary Care Clinics within Langley.
  • Develop Family Physician/Nurse Practitioner/Nursing recruitment strategies.
  • Manage physician call schedule and send out updates.

Communication and Liaison:


  • Build network relationships and act as a liaison with partners.
  • Manage locum pool communications and act as a locum contact for Family Physicians.
  • Conduct Long Term Care facility checkins and communication.
  • Possess strong writing and communication skills.
  • Manage social media communications.
  • Prepare and distribute Member News Bulletins.
  • Handle sensitive payment information and submissions.

Primary Care Network (PCN) Implementation:


  • Implement the Primary Care Network (PCN) and build new program structures.
  • Support for Integration Tables and Steering Committee (SC):
  • Compile and distribute meeting packages.
  • Follow up on action items.
  • Prepare slide decks and materials for presentations and meetings.
  • Translate information into visual formats.
  • Develop process maps and logic models.
  • Support the Primary Care Network (PCN) evaluation.

6. MedWatch Program

  • Manage collateral and updates.
  • Schedule presentations and track data.
  • Market and advocate for more presentations

7. Perform other duties as required

Qualifications and Competencies:


  • Minimum 1+ years' experience in a similar position.
  • Diploma/Degree in Communications, Health Administration, Business Management, Marketing, or Project Management preferred.
  • Experience in a memberbased organization or health administration setting is a bonus.
  • Access to reliable transportation for practice visits.
  • Experience with budgeting and monitoring project progress.
  • Managing tasks and time frames.
  • Familiarity with the processes of Health Match, Fraser Health Primary Care, FPSC, Doctors of BC and/or the College of Physicians and Surgeons is a bonus.
  • Works well as part of a team but maintains focus and initiative to work independently.
  • Effective and clear communication skills (written and oral).
  • Ability to work with given directions from the Project Lead.
  • Ability to use critical analysis skills to effectively and efficiently solve problems.
  • Proficient computer skills and ability to learn virtual platforms.
  • Experience with Microsoft 365, Teams, & Zoom platforms is an asset.
  • Proficiency in Excel/data management.
