1 ‐ Full Time - Nurse Practitioner Endoscopy/Surgical Central Intake and Assessment Navigator

Surgical Transitions Remote Pt Monitoring
Reporting to the Program Director and clinically to TBRHSC surgeons and gastroenterologists, the

Endoscopy/Surgical Intake and Assessment Nurse Practitioner Navigator provides leadership and an advanced level

of clinical practice within an area of gastroenterology and general surgery providing education to patients and clerical

staff; facilitating interdisciplinary processes across the organization; and working collaboratively with clients,

gastroenterologist, surgeons, managers and administration. The Surgical Central Intake and Assessment

Coordinator will provide central coordination for patients referred for surgical consultation.

The Surgical Central Intake and Assessment NP Navigator is responsible for the day‐to‐day clinical flow of the


Surgical Central Intake including:

  • Acting as the point of contact for triaging of all referrals to the program including liaising with Central Intake
Program and primary care referral sources

  • Supporting the clerical staff with bookings, referral sources, and re‐scheduling as needed
  • Collaborate with internal and external stakeholders to ensure ongoing evaluation and continued streamlining of
coordinated access

  • Supporting quality improvements and evidence‐based practice
  • Creates an environment that promotes collaborative teamwork, clinical inquiry, innovative thinking, constructive
debate and greater autonomy in clinical decision‐making

  • Monitoring and managing wait times for consultation and surgery/endoscopy for general surgery and
gastroenterology


RESPONSIBILITIES

  • Patient Care
conducts client initial assessment where appropriate

  • Facilitate the central intake process; present plan of care options to clients; refer to community resources; identify
client education needs for surgical/endoscopic optimization

  • Coordinate patient navigation and timely access to care across the North West
  • Ensures the triaging and booking of referrals to support equitable and timely access to care
  • Supports and provides direction to the clerical staff when needed to ensure timely access to care for patients
  • Continues with their direct patient care role to ensure clinical proficiency and knowledge of evidence based best
practices

  • Actively participates in the development of care pathways for patient groups to meet the program vision
  • Participates in development of policy and procedures related to patient care as required to reflect current evidence
and leading practices

  • Maintains professional development by participating in the mandatory programs of the hospital as well as attending
seminars and conferences to enhance treatment and operation of the program.

  • Facilitate evidence
- based practice/best practices at professional level; foster an environment of continuous

improvement and client centered care; facilitate development of inter‐professional practice initiatives; provide

education about the program and referral processes to local and district agencies/clinics/ practioners and other care

providers;

  • Develops processes and collect data relating to wait times and outcomes; shares information with colleagues,
funders and other interested parties;

  • Support, advise, counsel and educate patients regarding options, treatments and practices in the delivery of care;
  • Conduct interventions with and/or advocate on behalf of clients;
  • Determine clinical/professional educational needs; provide/coordinate programs;
  • Provide support and interventions to resolve issues and conflicts;
  • Contribute to clinical/professional/personal development through mentoring and coaching.
  • Assess and improve quality of client/system outcomes through evidence‐based practice.
  • Assess, develop, implement and evaluate process/practice improvement initiatives;
  • Contribute to and ensure the development and maintenance of policies, procedures, practices and performance
standards where appropriate.


QUALIFICATIONS

  • Unencumbered registration as a Registered Nurse in the Extended Class (EC) with the College of Nurses of
Ontario ‐ Adult specialty certificate Unencumbered registration as a Registered Nurse or Registered Nurse in the

Extended Class (EC) with the College of Nurses of Ontario

  • Minimum BScN with preference given to applicants who have completed or are close to completing graduate level
education in Nursing or administrative studies

  • Membership in NPAO or RNAO required
  • Minimum 5 years recent nursing experience in a clinical coordinator role
  • Demonstrated commitment to the principles of the Patient and Family Centred Care model
  • Clinical knowledge and expertise in endoscopy, general surgery and gastroenterology
  • Previous experience and passion for program development, implementation, data collection, and evaluation
