Description

The CPH Direct Support Worker is directly accountable for providing high quality support to people, working together with their support team and Home Support Leaders to ensure that goals and objectives are met.


Specifically, the Direct Support Worker is responsible to:

  • Provide direct support in a manner that is consistent with CPH philosophy and personalized support plans;
  • Follow through on team decisions, support plans and directions from their Home Supports Leaders and/or Home Supports Coordinator;
  • Participate in the development, support and delivery of personalized plans for the people they support, including attending meetings as requested;
  • Develop strong, positive relationships with the people they support, their families, cosupports and external agencies involved with the people CPH serves.
  • Participate in Person Centered Planning for people receiving support;
  • Attend training, team meetings, etc... as requested/required;
  • Provide assistance to the Home Supports Leader in tasks as assigned.
  • Ensuring the support provided is of high quality, person centered, and in harmony with planning CPH philosophy and Gentle Teaching;

PRINCIPLE FUNCTIONS AND ACCOUNTABILITY:

  • Provides direct support and life style planning consistent with CPH beliefs and values and Gentle Teaching;
  • Provides direct support to people relevant to their personal plan and contract expectations;
Provides direct support that reflects the Gentle Teaching Principles, in that:

a. Intent is not in changing others behavior but in changing ourselves

b. Support is always focused on teaching the person to feel safe and loved and become more loving and engaged

c. Positive use of the 4 tools in everyday support (hands, eyes, words, presence)

Support will include:

Providing support to someone who experiences intellectual disability, in conjunction with mental illness:


  • Providing support to someone who may experience physical and verbal aggression:

Providing support to someone who engages in self-harm and suicidal ideation:


  • Assisting someone in problem solving or decision making:
-
Providing assistance to someone in completing daily tasks (i.e. cooking, cleaning, etc...)

Sharing and/or developing options for community involvement or recreation:


  • Participating in team meetings and providing input, suggestions or ideas:

Medication administration in a manner consistent with the CPH medication policies and procedures:


  • Liaising with health care professionals as needed regarding people's needs:

Maintaining valid First Aid and CPR (Level C) certification and be willing to provide First Aid assistance if necessary

Expectations of duties:


  • Completes all required paperwork
  • Provides support consistently as scheduled
  • Ensuring that all matters relating to the support and care provided to the people served by CPH shall be treated with the utmost confidence and not discussed with people outside of the support team or in the presence of other people being supported by CPH.

PREFERRED QUALIFICATIONS:

  • Prior experience providing direct "hands on" support with people who experience intellectual disability, mental illness and/or a combination of both
  • Related postsecondary education is an asset;
  • Direct experience with or knowledge and belief in CPH philosophy and Gentle Teaching;
  • Valid Class 5 driver's license;
  • Strong organizational and communication skills;
  • Basic computer knowledge and skills including ability to work with Microsoft Office & Outlook
  • A working knowledge of the following:
  • Crisis Prevention and Intervention techniques
  • Alternative communications
  • Intellectual and/or developmental disability
It is desirable that this position sees the importance of the individually oriented support plans for each person served. This position should also demonstrate both creativity and common sense when serving people.
