Description of Duties and Responsibilities


The Multi-Program Licensed Practical Nurse (LPN) will provide direct client care to the Medically Supported Detox, George's House Palliative Care, and the Supervised Consumption Service.

The LPN will support clients who are in various stages of substance use with a focus on harm reduction and client safety.

The Multi-Program LPN will provide coverage to all three programs as needed including prescheduled and short notice shift work and will provide safe, quality patient and family centered care while reflecting the shared vision and values of the George Spady Society.


Medically Supported Detox


The MDU LPN will provide direct nursing care and support to clients who are in the process of detoxification, stabilization and/or transitioning into other residential addiction services while effectively challenging clients to ensure their safety and well-being.

The LPN will collaborate with an interdisciplinary team and will utilize nursing processes, critical thinking, problem solving and decision making.

The Detox LPN will determine the nature and severity of clients presenting to the Detox program and will be responsible for gleaning medical information and pertinent psychiatric information from electronic and paper files.

The LPN will be responsible for referrals, pre-screening, admissions, and medical care.


George's House


The LPN will be responsible for providing daily assistance as needed in a harm reduction and palliative care model which will include medication administration, wound care, enhancing quality of life, managing symptoms, end of life care and wishes of the clients loved ones.

The LPN understands and respects clients wish to die in comfort, with privacy and dignity at the last phase of one's life and has a good understanding of the different stages clients and their families will experience throughout out the illness and ending stages.


Supervised Consumption Service

Position Requirements

  • Excel in medication administration practices including oral, subcutaneous, intramuscular, suppository and high impact/narcotic medications.
  • Provide quality bedside assessment and care to individuals withdrawing from substances.
  • Conduct thorough health needs assessments and triage assessments to determine access to care.
  • Complete preinjection and postinjection assessments and monitors client wellbeing for the duration of their SCS visit.
  • In collaboration with the Registered Nurse, respond to overdose and other emergency requirements, when necessary (e.g., naloxone, oxygen, ventilation, CPR).
  • Provide case management and coordination as needed.
  • Liaise with primary care for clients.
  • Provide client centered and trauma informed care.
  • Work collaboratively with individuals, health care professionals and community services in the identification and resolution of health issues and integration of care, making referrals to other services providers as appropriate.
  • Maintain standards of nursing practice and develops professional competence through training and educational opportunities.

Qualifications

  • A minimum of a diploma in Nursing, with 3 to 5 years of experience providing health services to clients.
  • A flexible schedule to meet the needs of all three programs.
  • Good standing and registration with regulatory body.
  • Knowledge of CALPNA standards for Nursing Practice.
  • Demonstrated knowledge of palliative care.
  • Developed skills in conflict resolution and deescalation techniques.
  • Demonstrated knowledge of OAT and other Addiction Medication best practices.
  • Knowledge of community and outpatient resources available to inner city community members.

Days, weekends, various shifts
