About the Position:
Under the direction of a Program Manager, the Program Coordinator is responsible for supporting direct care staff (e.g.

Program/Family/Housing Support Workers) and contracted service providers (e.g., Foster Parents,) through pre-service training, in-service training, coverage, treatment planning, routine performance evaluations, TFA evaluations and facilitative administrative practices.

The Program Coordinator is accountable for treatment implementation program for all children, young adults and families served by the program.


Key Responsibilities:


  • Provide inservice training and ongoing consultation and feedback to ensure the implementation of the skills taught during preservice training, and to provide assistance in solving difficult problems and crisis situations. This is done through completion of the Closer to Home monthly service delivery system.
  • Participate in conducting routine performance evaluations to assess staff skills in providing treatment, the clients' opinions concerning the care and treatment they have received, and the satisfaction of a variety of consumers with respect to the treatment outcomes and the methods used to produce those outcomes.
  • Work cooperatively with practitioners to select appropriate services for families and maintain cooperative relationships with a wide variety of individuals and agencies.
  • Provide intensive back up coverage. Responsible for covering staff transitions and vacancies.
  • Attend all significant meetings where decisions are made that may impact treatment. Liaison with the appropriate groups to ensure that uptodate information is available to assist with appropriate planning.
  • Represent the best interests of the youths, young adults and families and advocate for them in situations that affect their lives.
  • Recruit and hire practitioners and help prepare them for professional certification by Closer to Home Community Services.
  • Provide services to children, young adults, and families in accordance with Closer to Home Community Services Standards for Quality Care and the policies and procedures of the Agency.
  • Ensure that the program complies with relevant CARF and TFA Standards.
  • Participate in program evaluation activities that help develop coordinator skills and ensure that the programs remain with a focus of continious improvement.
  • In general, behave in ways that promote cooperation and harmony and foster a positive, optimistic environment for children, young adults, families, staff and consumers.
  • Provide services to children and families in accordance with Closer to Home Community Services (TFA) Standards for Quality Care and the policies and procedures of the Agency.
  • To become a qualified trainer and evaluator within the first year as a coordinator.
  • Maintain and adhere to all health and safety requirement and report all hazards and injuries to immediate supervisor.
  • Proficiency in HMIS System and/or willingness to learn how to use this information system
  • Participate in the oncall rotation as directed and/or required

About You:


  • Relevant experience working with Indigenous peoples and communities is an asset
  • No history of abusing children, criminal record, or other problems that might indicate a lack of positive regard for children and families.
  • Ability to model appropriate behaviors, display good morals and values, and exercise good judgment in working with children, young adults, families, colleagues, and consumers.
  • Willingness to participate in skilledbased training programs and in regularly scheduled evaluations of professional skills and personal progress.
  • A valid driver's license and reliable transportation.
  • Personal vehicle insurance with liability insurance of a minimum of $1 million.
  • Will be aware of the importance of cultural competency and conduct themselves in a manner which is respectful and encouraging of the cultural/spiritual beliefs and practices of others.
