The University of Alberta Hospital is one of Canada's leading clinical, research, and teaching hospitals, treating more than 700,000 patients annually from western and northern Canada.
The hospital offers a wide range of diagnostic and treatment services, including specialized adult services within cardiac sciences, neurosciences, surgery, medicine, orthopedics, renal, critical care, burns, transplant and trauma inpatient programs as well as a vast number of ambulatory clinics.
This position is part of the centralized Rehabilitation and Allied Health Program at the University of Alberta Hospital and is located in Wetaskiwin.
This social worker will work with team members within Alberta Kidney Care North to support patients who receive service at the Wetaskiwin site with service provided virtually to patients in Drayton Valley and Vermillion.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families
Your services may include:
psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:
Preferred Qualifications:
Recent experience in acute care. Experience working with inpatients and ambulatory patients with chronic disease. Renal program experience preferred. Proficiency with Microsoft Office including Word, Excel, Outlook and PowerPoint. Familiarity with Netcare, Connect Care, NIS and principles of electronic documentation. Familiarity with Connect Care, Net Care and Nephrology Information System (NIS). Master's Degree is an asset.
