Role Summary_

Reporting to the Navigator Team Lead and the Associate Director, Youth Support Workers are responsible to assist youth (ages with a safe and supportive environment in the Emergency Shelter Program and the Dorms.

These programs support increased independence, allow youth to reconnect with important people in their lives, and to function more successfully in the community, all while residing in a safe and supportive housing environment.


Primary duties include collaborative teamwork, case management, crisis management and the safety of the Youth accessing programs and other staff working at Home Base.

Responsibilities_

  • Actively support the guidelines, program goals and expectations of Youth in Transition Program and the Emergency Shelter
  • Review Youth files at the beginning of every shift, going back to last shift worked or not less than one week to be aware of the current youth accessing services and the overall environment
  • Complete intake process for Youth into the shelter which is open 8pm12pmIncludes assessment of the immediate wellbeing of the youth, ensure all personal belongings are secured in locker prior to entry, meet immediate needs including safety and access to food and shower
  • Engage Youth who are Dorm residents in activities, conversation and ensure their safety and well being on return to Home Base after being in the community
  • Support the Key Navigator to engage Youth involvement in the development of service and transition plans with an emphasis on skill development (i.e. life skills, communication, problem solving, budgeting, and behavior management) in organic teaching situations to meet individually established outcomes
  • With the support of the Key Navigator, assist:
  • Youth in the maintenance of personal standards of care;
- overcoming dependencies, adjusting to life situations, and making positive change;
- the navigation and coordination of necessary support services;
- the resolution of emergency problems in crisis situations;
- increasing their capacity for autonomy, independent decision-making and problem solving;

  • Assist youth to express their feelings and discuss what is happening in their lives, and help them to develop insight to facilitate selfawareness and discovery
  • Attend and participate in as needed meetings with Overnight Team Lead
  • Document and communicate observed changes in Youth mood and/or behavior, health and/or mental health care needs
  • Facilitate effective communication with youth, families and other HomeBase staff regarding planning, development and support of youth program goals and outcomes
  • Participate in the maintenance of a clean common spaces within the Home Base locations throughout the shift to provide a clean homelike environment for the youth
  • Adhere to the Child and Family Services Caregiver Discipline Agreement
  • Ensure cultural and spiritual beliefs of all Youth are supported
  • Ensure a safe space for all, including 2SLGBTQQIPAA+ individuals
  • Respond to youth with empathy and compassion
  • Provide role modeling for Youth to aspire to achieve
  • When there are either no Youth in house or while in their dorm rooms/sleeping, complete other required responsibilities including additional duties as requested by the Associate Director, your immediate supervisor or the Executive DirectorThese duties include simple maintenance in the building, organizing, cleaning, and keeping signage and information posters up to date

Crisis Management_

  • Recognize potential emergency situations and intervene as necessary, including calling 911 for assistance
  • Analyze situations to mitigate risk to the Youth, other Youth and/or staff
  • Ensure safety risks are identified and the appropriate intervention is applied
  • Complete safety plans with Youth when appropriate
  • Work directly with the Youth to develop strategies to deal with potentially dangerous situations; i.e., suicide threats, AWOL's, youth's aggressive behavior
  • Assess, evaluate and plan to prevent crisis situations whenever possible
  • Demonstrate knowledge and understanding of the importance of safety precautions dealing with environmental working conditions such as potential verbal and physical aggression
  • Demonstrated capacity to implement suicide risk assessments, appropriate safety planning and response
