Clinical Pharmacy Specialist/ Leader: Drug use evaluation (0.5FTE)


The Clinical Pharmacy Specialist/ Leader (CPS/L), assignment as Drug Information/Drug use evaluation, serves in the role of a leader for medication system of the network.

This role, based at SMH but also integrates with other sites, is highly linked with the functions of the Pharmacy Department and pharmacy practice, education and technical operations.

In alignment with the general job description for Clinical Pharmacy Specialist/ Leader, the individual fulfilling this position will:


  • Provide leadership in networkwide appropriate usage of medications on a macro level
  • Act as lead/secretary of the networkwide Pharmacy and Therapeutics committee, in conjunction with other relevant CPS/L
  • Collaborate with other relevant CPS/L to maintain Drug Information service for hospital staff and physicians as well as to maintain uptodate resources
  • Lead in maintenance of networkwide drug formulary and other associated databases (e.g. IV pump libraries
  • To proactively lead the evaluation, development, revision and implementation of medication related standards, protocols, guidelines, procedures typically with crossprogram or hospitalwide focus; and to promote desired outcomes through education, internal publication and monitoring.
  • Provide leadership and mentorship to clinical pharmacists, pharmacy students when assigned to assist in medication systemrelated projects or in answering drug information questions
  • Participate or lead committees affecting medication systems such as Order set committee
  • Support departmental or corporate initiatives in teaching and/or research, quality improvement, workload documentation, service planning, and procurement related functions as assigned
  • Participate in scholarly work such as internal/external publications, drug use evaluations
  • Precept pharmacy students and residents on evidencebased medicine, drug information and drug use evaluation

QUALIFICATIONS

  • Advanced degree in Pharmacy (Doctor of Pharmacy preferred) or equivalent in terms of practice experience
  • Registration as a pharmacist with the Ontario College of Pharmacists (Part A Register)
  • Minimum of eightyears pharmaceutical care practice experience in a teaching hospital as well as demonstrating progressive degrees of teaching responsibility
  • Excellent drug information skills
  • Strong leadership skills, demonstrating progressive degrees of responsibility and the ability to lead projects, people and corporate committees
  • Demonstrated skills, knowledge and ability in all the following:
oProject management at either departmental or higher level

oMentoring and teaching skills development

oProblem solving in a rapidly changing environment

  • Experience in or demonstrated potential for scholarly activity
  • Excellent interpersonal, change management and facilitation skills and strong customer focus
  • Excellent ability to be focuses, detailoriented, and communicate verbally/in writing to diverse audiences
  • Demonstrated commitment to the vision, mission and values of Unity Health Toronto Hospital, and to the philosophy of pharmaceutical care
