    Description

    Responsibilities:

    • Provides public health nursing service for individuals, families, groups and the community according to specific policies and procedures.
    • Conducts relevant screening and health/service assessment at various levels and develops appropriate plans.
    • Makes referrals to other health services and community resources.
    • Delivers interventions focusing on health promotion and behavior change.
    • Maintains service delivery records and necessary documentation.
    • Collaborates with community groups, agencies, and professionals to assess health needs.
    • Facilitates community partnerships and liaises with various stakeholders.
    • Represents the organization on community networks and committees.
    • Provides training and support internally and externally.
    • Prepares and presents activity reports and data for managerial approval.
    • Participates in professional conferences and workshops.
    • Contributes to the development and implementation of health promotion programs and resources.
    • Participates in divisional planning processes and project coordination.

    Key Qualifications:

    • A Baccalaureate degree in nursing and a valid license from the College of Nurses of Ontario.
    • Experience in public health or community nursing.
    • Proficiency in French at a professional level.
    • Possession of a valid Class 'G' Ontario Driver's License and access to a vehicle.
    • Knowledge of population health approach and health promotion theory.
    • Strong oral and written communication skills in both languages.
    • Effective counselling and group facilitation skills.
    • Interpersonal skills for maintaining public relations.
    • Ability to work using inclusive and harm reduction practices.
    • Capability to provide services to diverse cultures and communities.
    • Effective team collaboration and community partnership skills.
    • Flexibility to work varied hours and travel within the city.
