    Unit Summary

    The 2F Digestive Diseases clinic provides ambulatory care and services within a multidisciplinary team for client with a variety of disorders. Our patient population includes patients with medical and surgical gastrointestinal disorders. The Inflammatory Bowel Disease (IBD) clinic within 2F, supports patients with chronic IBD experiencing acute symptoms, by providing, support, defined management goals, education and progressive management for individual therapies.

    Position Summary

    The Nurse Practitioner (NP) - Gastroenterology, is part of a dynamic care team that includes Gastroenterologist, Registered Durses, Dietician, Research Coordinators, and Enterostomal Therapy Nurse. The team works seamlessly with an extended interdisciplinary team to diagnose, treat and advance the quality of care for complex chronic gastrointestinal disorders conduct research in the adult population. In collaboration with the multidisciplinary team, the NP will apply advanced decision making skills in health assessment, diagnosis, therapeutics and healthcare management, with a focus on clients affected by chronic, complex gastrointestinal disorders; these will be, primarily, clients with inflammatory bowel disease (IBD), including those with complex disease.
    In addition to direct clinical care, the NP functions fully in the 5 domains of the role identified within Canadian Nurses Association's APN Framework. These domains consist of the clinical practice, research, leadership, consultation and collaboration with education. The NP will provide leadership and promote excellence in patient centered nursing care through expert clinical practice, education, consultation and research. The NP will liaise with the multidisciplinary team and will support clinical outpatients of IBD clients. These supports include the telephone advice line, NP lead outpatient/telephone clinic, rapid access clinic, managing disease flares, assessment leading to investigation and treatment, providing education and planning for advance IBD therapy, coordinating colorectal cancer surveillance for IBD clients, managing pediatric to adult transition pathway, service development and leadership, mentoring healthcare learners.

    Qualifications

    1. Current registration with the College of Nurses of Ontario, as a Registered Nurse (Extended Class) RN(EC)
    2. Successful completion of an Acute Care Nurse Practitioner - Adult program, in Ontario or equivalent
    3. Masters Degree in Nursing and/or equivalent graduate Degree
    4. Recent (5 years) clinical experience with Medical/Surgical GI Disease management
    5. Experience in developing, implementing and evaluating nursing practice tools in an ambulatory setting
    6. Current CNA Certificate in Gastroenterology nursing is preferred?
    7. Extensive knowledge of evidence based practice in the care and management of adult patients with complex GI diseases (ie: IBD)
    8. Demonstrated commitment to patient and family-centred care
    9. Demonstrated ability to foster collegial relationships within and among the hospital, clinics and community
    10. Experience in leading inter-professional research initiatives and publishing outcomes
    11. Demonstrated excellent independent, teamwork, interpersonal and communication skills
    12. Expertise in strategies to improve patient adherence, manage outcomes of chronic disease, create and implement policies, procedures and medical directives, utilizing evidence-based standards of practice and best practice guidelines, where applicable, is required
    13. Demonstrated commitment to continuous learning and the ability to promote professional development in nursing
    14. Demonstrated ability to be flexible and adapt positively to constantly changing clinical situations
    15. Eligible for faculty appointment at McMaster University School of Nursing
    16. Membership in provincial, federal or international professional associations
