Duties & Responsibilities

Comprehensive Community Health Programming & Service Delivery
Communicable Disease Control (CDC): Assures comprehensive CDC program including focus on influenza HIV Hepatitis A B C STIs Tuberculosis TB vaccine preventable diseases Provides community awareness programming on immunizations other communicable diseases prevention early detection transmission control strategies Plans implements evaluates immunization programs for infants children adults elders members deemed higher risk Ensures required training completed competency maintained current immunization certification transportation dangerous goods TDG CPR infant certification Ensures regular maintenance CDC equipment BCCDC recommended CDC supplies available Ensures cold chain principles adhered to Ensures mandatory CDC reporting completed timely fashion Maternal Child Family Health: Assesses needs strengths resources plans implements evaluates prenatal postnatal newborn programming linkage Canadian Prenatal Nutrition Programming Provides prenatal postnatal newborn education support programming topics injury prevention SIDS nutrition lifestyle health practices growth development prenatal care newborn care breast feeding physical emotional care seeks medical attention Offers referrals other health professionals programs required Adult Elder Health: Assesses needs strengths resources plans implements evaluates adult elder programming collaboration Home Care Nurse Community Health Representative Elders Coordinator Topics healthy lifestyle early detection illness emotional wellness women's men's health topics injury falls prevention elder abuse neglect prevention Refers other health professionals agencies appropriate Chronic Disease Prevention Management: Applying integrated team approach coordinates Chronic Disease nurse HCCC assesses needs healthcare Immunization certification willingness complete within months hire Minimum four years experience community nursing home care nursing Minimum three years primary care hospital setting Food Safe Certificate required Ability lift pounds perform physical tasks Current CPR infant certification willingness obtain first months Commitment continuing professional development Alignment ethical
