Community Support Worker

General Job Description:


The Community Support Worker will assist clients to meet their needs and enable them to utilize agency or community services and supports to achieve their optimal level of health and functioning.


Qualifications:

Completion of a Secondary School Diploma required; post-secondary education in a health or social services discipline is an asset. The combination of work experience, life experience and education will be considered as part of the necessary credentials.

Of most importance to this position is an outlook and demeanor that is conducive to meeting individuals 'where they are at' and leveraging natural and formal support systems as required.


Job Status:

Full time position. This position is contingent upon ongoing approved funding. A valid driver's license and access to a road worthy vehicle are requirements for this position.


Salary:
$42, $50,483.00 per year


Responsibilities:


  • To provide support to clients and/or their families who have selfidentified or have been identified by a community provider as requiring case management, practical assistance, service system navigation, referral or other services/supports either in person, on the telephone or via a virtual platform
  • Flexibility to work beyond regular agency hours when required
  • Flexibility to work outside of the office setting and remote as required i.e., hospital, partner agencies, client homes etc.
  • To provide support for and enhance treatment planning options
  • To refer for social, psychological, emotional, and family problems
  • Understand and respond to a variety of situations and conditions, tailoring responses appropriately
  • Function as a resource for community and continued support programs, providing accurate information as required
  • Report to authorities and take other protective action when required in cases of child abuse and/or neglect
  • Adhere to and work within all organizational, federal, and provincial guidelines and standards related to privacy and confidentiality
  • To develop and maintain sustainable working relationships with community agencies, schools, hospital, medical professionals, law enforcement, courts etc.
  • Maintain and model appropriate personal boundaries with clients
  • To utilize appropriate resources from community, social, health and welfare agencies
  • Provide advocacy when appropriate
  • Provide referrals to services as appropriate
  • Refer for safety planning related to suicide; domestic violence etc. as required
  • Consult as appropriate with supervisor, community partners, families, friends, and support networks
  • To maintain accurate client records and statistical reports
  • To maintain the confidentiality of all client information
  • To engage in case management and case referral with other community organizations
  • To engage in regular supervision with the Executive Director or Clinical Manager
  • To attend and participate in staff and agency meetings to work as a team member
  • Use a variety of technology to document all work; proficiency with computers is required.
  • Perform all other duties that may be required from time to time as requested by the Executive Director or Clinical Manager.
