QUALIFICATIONS:
- Undergraduate degree in human or social sciences or equivalent, with minimum 3 years related experience
- Direct experience working with people who have developmental and physical disabilities and their families or other networks
- Highly developed interpersonal and problem solving skills
- Good supervision skills and experience
- Excellent organizational and time management skills, oral and written communication skills
- Verbal fluency in multiple languages as an asset
- Verbal fluency in Arabic as an asset
- Ability to multitask and to work well under pressure, both independently and as part of a team
- Mental health experience and education as an asset
- Knowledge of local community resources and services as an asset
- Access to a reliable vehicle
HOURS AND PAY:
- 37.5 hours per week. Hybrid remote, two days per week inperson. Regular schedule flexible in discussion with supervisor.
- Salary Band 7: Starting rate $26.41 to $28.01 per hour.
- Benefits enrollment after 3 months of regular employment.
ROLE:
The Support Coordinator - Adult Services is responsible for supporting individuals living with developmental disabilities to utilize and navigate provincial Passport funding they receive.
RESPONSIBILITIES AND FUNCTIONS
Reporting to the Program Manager, ensures the provision of services offered by Extend-A-Family Waterloo Region to individuals with developmental and/or physical disabilities and their families:
Provide Support to Individuals and their Families:
- Support individuals and families to identify their current support needs
- Assist individuals and families in the development of a support plan using allocated or available funding
- Assist individuals and families in monitoring both the available funding and the plan, facilitating adjustments as requested
- Identify and introduce DSPs to individuals and families
- Provide ongoing support to both DSPs and individuals/families to maintain a positive working relationship
- Support individuals and families to access additional community supports and resources as requested
- Liaising with and creating opportunities with other community organizations, services and businesses
- Complete reports and updates for agency partners
- Disseminate agency information as requested
Provide support to Direct Support People (DSPs):
- Assist in the orientation of new DSPs to the organization
- Identify individuals/families that may fit for the DSPs skill set and availability and facilitate introductions
- Work with Recruitment Coordinator, as required, to access specific DSPs for contracts
- Provide DSPs with specific individual and family orientation, expectations and resources
- Complete regular checkins with DSPs
- Assist DSPs in addressing issues and concerns related to their work
- Facilitate development opportunities for DSPs as requested
Prepare Reports:
Completion of the following on a regular and timely basis:
DSP file updates including contact notes, incident reports, unusual incident or serious occurrence reports and copies of correspondence as appropriate
- A complete file of the individual, including case notes
- Caseload, match status and other statistical reports as required
- Website content as requested
- Special reports as requested
Attend Meetings:
- Attend supervision meetings with the Program Manager as requested/required
- Attend weekly meetings focused on team and organizational updating and development
- Participate in internal/external committees with approval and direction of the organization
Special Assignments and Other Duties:
- Undertake key activities and new ventures, in conjunction with or as assigned by the Director of Programs, Organizational Development Director and/or Executive Director.
- Promote growth of an organizational culture that is both inclusive and antioppressive.
