Job summary
The Case Manager will co-ordinate a support network and links clients living with mental illnesses addictions and physical health issues in Toronto Transitional Age Youth Services.
Responsibilities
- Develop a plan of support recovery including identifying priorities goals in conjunction with client other professionals areas identified through initial intake assessment;
- Promote personal empowerment of each individual through coaching mentoring support thereby enabling each to meet daily requirements of their living environment directly assisting with daily living activities particularly in acquisition of life skills hygiene medication management cooking cleaning shopping budgeting develop strategies establish plan for maintaining these daily activities;
- Establish coordinate support network social family medical psychiatric community links develop coordinated care plan as appropriate provide crisis intervention management assist development crisis plans with clients promote services program image agency representative clients families supports external agencies others professional manner reflecting favourably on organization participate planning organizing promoting conducting events for clients effort address social needs prevent isolation maintain general awareness community events activities contribute program delivery providing support consultation colleagues identifying gaps service recommending solutions participating meetings planning evaluation initiatives committees events activities orienting new staff students volunteers related assistance required encourage participation one one group opportunities supporting goals help reduce social isolation positively contribute TAY program engaging collaborative team activities supporting colleagues assist preparing units occupancy including moving furniture
