Therapeutic Family Lead


The Therapeutic Family Lead (TFL) is responsible for case management, coordination, supervising, developing, implementing, and evaluating the delivery of services to young people and their families.

The TFL provides leadership and case management support to the therapeutic team that will serve to optimize the relationship between the young person and their family.

The TFL is also responsible for the provision of support in the therapeutic home and community support within the scope of the organizational resources.

The TFL also provides consultant services to staff and assists in the coordination of services to families of the youth we serve.


Hours:
This position is a Monday-Friday position. You will create your schedule based on the family's needs and with your colleagues and Program Director. There is also an on-call component that will be discussed in greater detail during the orientation.

The specific TFL responsibilities include the following:

A. Oak Hill Foundation Philosophy, Policy, Procedure and Organizational Skills.

  • Consistently utilize Oak Hill Foundation policy and procedural manual to guide decision-making and action.
  • Demonstrate knowledge of organization service philosophy and service framework in decision-making.
  • Can describe organizational structure, positions, roles and decision-making process.
  • Understand the job description and performance expectations. Take initiative in promoting organization, program, and professional development.
  • Establish and maintain relationships with organizational staff based on respect, genuineness, commitment, and integrity.
  • Demonstrate efficiency and effectiveness in the use of work time and performance expectations.
  • Work as a member of the program team and provide input to the organization's leadership team. Present information and ideas on major issues and case consultations. Opinions are presented in an objective and articulate manner. Listen and respect the opinions of others and integrate input in a respectful manner as well as follow through with team decisions.
  • Perform responsibilities in an ethical manner, maintaining respectful conduct.

B. Case Management
a. Establish and maintain a helping relationship based on foundation helping skills of respect, genuineness, warmth, and empathy.

b.

Active listening and communication skills of paraphrasing, reflecting, clarifying, and summarizing are consistently practiced with all the young people in the program.

c. Contact with families regarding daily activities, appointments, incidents, and events.

d. Ensure daily documentation is completed.

e. Act in a professional manner with internal and external stakeholders.

f. Collaborate with community members.


C. Case Planning and Engagement

  • Plan, develop, implement, coordinate, and evaluate the delivery of services to children and their families.
  • Document daily activities and submit detailed reports on a monthly and quarterly basis.
  • Ensure that new admissions have the opportunity to participate in the comprehensive orientation involved in the program.
  • Ensure there is an integration of family services into the case planning needs of the young person.
  • Ensure goals have a meaningful connection to the young people's healthy development and their plan for permanency and are consistent with their therapy.
  • Monthly reports and Service Plans are completed in a timely manner with the participation of the family.
  • Attends and participates in relevant case conferences.
  • Ensures families have opportunities to participate in Ranch activities.
  • Plans, and coordinates regular family day activities.
  • Develops and facilitates parent groups that meet the needs of families receiving services

D. Care and Behaviour Management.

  • Be present within the therapeutic home and provide community-based support and prevention services to children and their families.
  • All behaviour management strategies will be consistent with policy and procedure, accreditation standards and best practice research.
  • All trauma management strategies will be consistent with policy and procedure, accreditation standards and best practice research.
  • Deliver programs that promote healthy lifestyles and relationships for families.
  • Promotes and encourages each family to maintain an optimum level of health, well-being and self-esteem.
  • Respects and protects the rights of clients, including but not limited to their right to independence, autonomy, and self-determination; their right to choose and practice individual values, beliefs, religion, and culture; their right to privacy and dignity; and their right to live free from retaliation, discrimination, and abuse.
  • Maintains and models appropriate personal boundaries with clients, and refrains from involvement with client personal affairs or other business that may constitute a conflict of interest.
